益气蠲饮方治疗胸腔积液的疗效及对炎症反应的影响研究

摘　　要：目的:研究益气蠲饮方的作用机理及其在胸腔积液治疗中的应用效果。方法:将某院收治的80例胸腔积液患者随机分为C组和C^(+)Y组各40例,C组采用积液病因对症治疗^(+)胸腔穿刺抽液治疗,C^(+)Y组在C组的基础上联合采用益气蠲饮方治疗。对比两组患者的临床干预效果;并于治疗前后,检测胸腔积液炎症指标[白介素-6(IL-6)、白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)]和免疫功能指标(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))。结果:治疗后,C^(+)Y组患者中医症候总积分低于C组(P<0.05);C^(+)Y组患者临床干预有效率为90%,高于C组的75%(P<0.05)。治疗后,C^(+)Y组患者胸腔积液IL-6、IL-8、TNF-α均低于C组(P<0.05),C^(+)Y组患者CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均高于C组(P<0.05)。结论:益气蠲饮方治疗胸腔积液的疗效显著,可有效抑制胸腔积液炎症反应,提高患者免疫功能。Objective:To study the mechanism of Yiqi juanyin decoction and its application effect in the treatment of pleural effusion.Methods:80 patients with pleural effusion admitted to a hospital were randomly divided into group C(n=40)and group C^(+)Y(n=40).Group C received symptomatic treatment of effusion etiology^(+)pleural puncture and drainage;Group C^(+)Y was combined with Yiqi Juanyin therapy on the basis of group C.The clinical intervention effect was compared between the two groups.The inflammatory indexes[interleukin-6(IL-6),interleukin-8(IL-8),tumor necrosis factor-α(TNF-α)]and immune function indexes(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))were detected before and after treatment.Results:After treatment,the total score of TCM syndrome in C^(+)Y group was lower than that in C group(P<0.05).The effective rate of clinical intervention in C^(+)Y group was 90%,which was higher than that in C group(75%,P<0.05).After treatment,pleural effusion IL-6,IL-8 and TNF-αin C^(+)Y group were lower than those in C group(P<0.05),and CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in C^(+)Y group were higher than those in C group(P<0.05).Conclusion:The effect of Yiqi Juanyin decoction in the treatment of pleural effusion is remarkable,which can effectively inhibit the inflammatory response of pleural effusion and improve the immune function of patients.
