摘  要:目的 研究肺腺癌组织中转化生长因子-α (TGF-α)、抑癌基因(p53)、细胞核增殖抗原(Ki-67)、上皮钙黏蛋白(E-cadherin)表达水平及其相关性。方法 收集2018年1月至2020年12月期间安康市中心医院手术切除的87例肺部肿瘤标本作为观察组,根据组织病理学的分级增高顺序将上述87例患者分为原位腺癌(AIS,35例),微浸润性腺癌(MIA,23例)和贴壁生长为主的浸润性腺癌(LPA,29例),并选择其中的87例肺部肿瘤标本的癌旁正常肺组织作为对照组。采用免疫组化S-P法检测肺部肿瘤及正常肺组织中TGF-α、p53、Ki-67以及E-cadherin的表达,比较四种蛋白在肺腺癌组织和正常肺组织的阳性率及在AIS、MIA及LPA的阳性率。采用Spearman分析法分析TGF-α、p53、Ki-67和E-cadherin蛋白的相关性。结果 观察组组织的TGF-α、p53、Ki-67及E-cadherin阳性率为59.77%、42.53%、48.28%、36.78%,正常肺组织分别为20.69%、19.54%、19.54%、90.80%,四种蛋白在肺腺癌组织和正常肺组织中的表达水平比较差异均具有统计学意义(P<0.05);TGF-α在AIS、MIA及LPA中的阳性率分别为42.86%、78.26%和86.21%,p53的阳性率分别为25.71%、47.83%和68.97%,Ki-67的阳性率分别为34.29%、52.17%和72.41%,E-cadherin的阳性率分别为48.57%、47.83%和20.69%,四种蛋白在AIS、MIA及LPA组织的表达水平比较差异均具有统计学意义(P<0.05);经Spearman相关分析结果显示,肺腺癌组织中TGF-α、p53及Ki-67表达与E-cadherin均呈负相关(r=-1.984、-2.045、-2.320,P<0.05)。结论 相较于正常肺组织,TGF-α、p53、Ki-67、E-cadherin蛋白在肺腺癌组织中表达异常,联合检测这四种蛋白可能有利于肺腺癌的早期诊断、分型和预后。Objective To study the expression levels of transforming growth factor-α(TGF-α),tumor suppressor gene p53(p53),nuclear proliferation antigen Ki-67(Ki-67),and epithelial cadherin(E-cadherin)in lung adenocarcinoma tissues and their correlation.Methods Eighty-seven specimens surgically resected from 87 lung tumor patients in Ankang Central Hospital from January 2018 to December 2020 were collected as an observation group.According to the order of histopathological grading,the above 87 patients were divided into adenocarcinoma in situ(AIS,n=35),microinvasive adenocarcinoma(MIA,n=23),and adherent growth-based invasive adenocarcinoma(LPA,n=29),and 87 cases of normal paracancerous tissue in the same period were randomly selected as a control group.Immunohistochemical SP method was used to detect the expression of TGF-α,p53,Ki-67,and E-cadherin protein in lung tumors and normal lung tissues,and the positive rates of the four proteins in lung adenocarcinoma tissues and normal lung tissues,and the positive rates in AIS,MIA,and LPA were compared.Spearman analysis was used to analyze the correlation between TGF-α,p53,Ki-67 and E-cadherin proteins.Results The positive rates of TGF-α,p53,Ki-67,and E-cadherin in the observation group were 59.77%,42.53%,48.28%,and 36.78%,respectively,and those in normal lung tissue were 20.69%,19.54%,19.54%,and 90.80%,respectively.There were significant differences in the expression levels of four proteins between lung adenocarcinoma and normal lung tissues(P<0.05).The positive rates of TGF-αin AIS,MIA,and LPA were 42.86%,78.26%,and 86.21%,respectively,the positive rates of p53 were 25.71%,47.83%and 68.97%,respectively,the positive rates of Ki-67 were 34.29%,52.17%,and 72.41%,respectively,and the positive rates of E-cadherin were 48.57%,47.83%,and 20.69%,respectively.There were significant differences in the expression levels of the four proteins in AIS,MIA and LPA(P<0.05).Spearman correlation analysis showed that the expression of TGF-α,p53 and Ki-67 in lung adenocarcinoma tissue were ne
