英夫利西单抗联合硫唑嘌呤对中重度炎性反应性肠病临床效果、营养状况及对CRP、ESR、TNF-α的影响

摘　　要：目的:观察英夫利西单抗(IFX)联合硫唑嘌呤治疗中重度炎性反应性肠病(IBD)的临床效果。方法:选取收治的中重度IBD患者78例,按不同给药方法分为观察组和对照组各39例。两组均给予常规药物治疗,对照组加用硫唑嘌呤治疗,观察组则给予IFX联合硫唑嘌呤治疗,观察并比较两组疾病活动指数(DAI)、炎症指标、营养状况。结果:治疗22周后与治疗前相比,两组DAI均降低,且观察组明显低于对照组,差异有统计学意义(P<0.05)。观察组治疗有效率94.87%高于对照组的82.05%,差异有统计学意义(P<0.05)。治疗后观察组CRP、ESR、TNF-α均明显低于对照组,营养水平指标血红蛋白、白蛋白、转铁蛋白均显著高于对照组,差异均有统计学意义(P<0.05)。结论:IBD采用IFX联合硫唑嘌呤治疗能够显著减轻患者炎症反应,提高患者营养状况,提高治疗效果,临床效果优于单用硫唑嘌呤。Objective To observe the clinical value of infliximab combined with Azathioprine in the treatment of moderate to severe Inflammatory Bowel Disease(IBD).Method A total of 78 patients with IBD were selected as the study subjects.According to different administration methods,they were divided into observation group and control group,39 cases in each group.The two groups were given routine treatment.On this basis,the control group was treated with azathioprine,the observation group was given Infliximab(IFX)combined with azathioprine treatment.The clinical efficacy of the two groups was compared,and changes of Disease Activity Index(DAI),inflammatory factors and nutritional status.Results After 22 weeks of treatment,compared with before treatment,the DAI of the two groups were reduced,and the observation group was lower than the control group,the difference was statistically significant(P<0.05);The total effective rate of the observation group(94.87%)was higher than that of the control group(82.05%)(P<0.05).The levels of serum CRP,TNF-αand ESR in the observation group after treatment were lower than those in the control group(P<0.05).In contrast,the nutritional level indicators of hemoglobin,albumin,and transferrin in the observation group were significantly higher than those in the control group(P<0.05).Conclusion IFX combined with azathioprine are effective in the treatment of IBD,which can significantly reduce the level of inflammatory factors and improve the nutritional status of patients.
