摘　　要：目的分析阿加曲班联合丁苯酞软胶囊治疗穿支动脉脑梗死的临床效果。方法选择2017年6月—2020年6月间铜陵市人民医院神经内科诊治的76例穿支动脉脑梗死患者为研究对象,采用随机数字表法分为研究组(39例)与对照组(37例)。对照组在常规治疗基础上联合应用硫酸氢氯吡格雷片,研究组采用阿加曲班联合丁苯酞软胶囊。比较2组患者美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)评分、Barthel指数(Barthel index,BI)、进展性运动障碍(progressive muscular dystrophy,PMD)发生率、神经功能恢复(mRS评分)情况,采用世界卫生组织生存质量测量量表(World Health Organization Quality of Life-100,WHOQOL-100)评价生活质量。结果研究组与对照组12周时NIHSS评分分别为(4.1±1.7)分、(5.6±1.5)分,研究组显著低于对照组(P<0.05);BI评分分别为(76.9±16.3)分、(68.3±13.7)分,研究组显著高于对照组(P<0.05);2组患者mRS评分分别为(2.6±0.6)分、(2.9±0.8)分,差异无统计学意义(P>0.05)。12周时生活质量评价结果显示,研究组生理健康、社会关系、独立能力评分显著高于对照组(均P<0.05)。结论阿加曲班联合丁苯酞软胶囊对穿支动脉脑梗死患者具有较好的治疗效果,有较高的应用价值。Objective To determine the clinical effect of argatroban combined with butylphthalide soft capsules in the treatment of perforating cerebral infarction.Methods Seventy-six patients with perforating cerebral infarction were enrolled and randomly divided into the study group(n=39)and control group(n=37).The patients in the control group were given clopidogrel bisulphate tablets on the basis of conventional therapy,and those in the study group were given argatroban and butylphthalide soft capsules.The National Institute of Health stroke scale(NIHSS)score,Barthel index(BI),incidence of progressive muscular dystrophy(PMD)and neurological recovery(mRS score)were compared.The World Health Organization Quality of Life-100 was used to evaluate the quality of life.Results At the 12 th week,the NIHSS scores of the study group and control group were(4.1±1.7)points and(5.6±1.5)points,respectively.The study group had a significantly lower score(P<0.05).The BI scores of the study group and control group were(76.9±16.3)points and(68.3±13.7)points,respectively.The study group had a significantly higher score(P<0.05).The mRS scores of the study group and control group were(2.6±0.6)points and(2.9±0.8)points,respectively,with no statistical difference(P>0.05).The results of the quality of life evaluation showed that the scores of physiological health,social relations and independent ability of the study group were significantly higher than those of the control group(all P<0.05).Conclusion Argatroban combined with butylphthalide soft capsules has a good therapeutic effect and high application value on patients with perforating cerebral infarction.
