摘 要:目的探讨术前肠内营养对食管癌患者围手术期营养状态及免疫功能的影响。方法选取2019年1月至2020年12月安阳市肿瘤医院收治的280例食管癌患者,根据随机数字表法将其分为对照组和观察组,各140例。两组均接受食管癌根治颈部吻合术,观察组术前行肠内营养干预,对照组术前行常规饮食干预,两组术后均进行肠内营养干预并观察7 d。比较两组的干预效果。结果干预后,两组患者的前白蛋白(PA)、白蛋白(ALB)、转铁蛋白(TF)、免疫球蛋白G(IgG)、免疫球蛋白A(IgA)、免疫球蛋白M(IgM)水平均高于干预前,且观察组高于对照组(P<0.05)。干预后,两组患者的白细胞分化抗原3阳性(CD3^(+))、白细胞分化抗原4阳性(CD4^(+))及CD4^(+)/CD8^(+)高于干预前,白细胞分化抗原8阳性(CD8^(+))低于干预前,且观察组优于对照组(P<0.05)。观察组患者的切口感染、吻合口瘘、肺部感染、泌尿系统感染、全身炎症反应综合症发生率低于对照组(P<0.05)。结论术前肠内营养有利于改善食管癌患者围术期营养状态,增强免疫功能,改善预后。Objective To explore the effects of preoperative enteral nutrition on perioperative period nutritional status and immune function in patients with esophageal cancer.Methods A total of 280 patients with esophageal cancer admitted in Anyang Tumor Hospital from January 2019 to December 2020 were selected and divided into control group and observation group according to the random number table method,with 140 cases in each group.Both groups received radical neck anastomosis for esophageal cancer.The observation group received preopetative enteral nutrition intervention,and the control group received routine diet intervention before operation.Both groups received enteral nutrition intervention after operation and were observed for 7 d.The intervention effects of the two groups were compared.Results After intervention,the levels of prealbumin(PA),albumin(ALB),transferrin(TF),immunoglobulin G(IgG),immunoglobulin A(IgA)and immunoglobulin M(IgM)in the two groups were higher than those before intervention,and those in the observation group were higher than the control group(P<0.05).After intervention,the cluster of differentiation 3 positive(CD3^(+)),cluster of differentiation 4 positive(CD4^(+))and CD4^(+)/CD8^(+)in the two groups were higher than those before intervention,cluster of differentiation 8 positive(CD8^(+))was lower than that before intervention,and those in the observation group were better than the control group(P<0.05).The incidences of incision infection,anastomotic leakage,pulmonary infection,urinary tract infection and systemic inflammatory response syndrome in the observation group were lower than those in the control group(P<0.05).Conclusion Preoperative enteral nutrition can improve perioperative period nutritional status,enhance immune function and improve prognosis of patients with esophageal cancer.
