全科医师儿童支气管哮喘诊治策略

摘　　要：支气管哮喘是儿童时期最常见的慢性气道疾病。近20年来,儿童哮喘患病率持续上升,约30%的儿童哮喘未能得到及时诊断,20%以上的儿童哮喘未达到良好控制。由于病情持续,反复发作,严重影响儿童的健康、生活和学习,对家庭造成了很大精神压力和经济负担。支气管哮喘的防治强调长期、持续、规范、个体化治疗原则,早期诊断、规范化管理和干预是提高儿童哮喘控制水平和改善预后的重要手段。但在临床具体实施中,特别是在基层医疗机构还存在认识和经验不足。本文参照近年来国内外最新发表的哮喘诊治指南及共识,针对基层医疗机构和全科医生,阐述哮喘的诊治策略,希望有助于提高基层医生对儿童哮喘的临床管理水平,从而整体提高儿童支气管哮喘的控制水平,降低发病率。Bronchial asthma is the most common chronic airway disease in children.In the past 20 years,the prevalence of asthma in children has been increasing,and about 30% of children’s asthma has not been diagnosed in time,and more than 20% of children’s asthma has not reached good control.Due to the persistence and recurrence of the disease,it seriously affects children’s health,life and study,and causes great mental pressure and economic burden to the family.The prevention and treatment of bronchial asthma emphasize the principle of long-term,continuous,standardized and individualized treatment.Early diagnosis,standardized management and intervention are the important means to improve the control level and prognosis of children’s asthma.However,there are still insufficient knowledge and experience in clinical implementation,especially in primary medical institutions.This article expounds the diagnosis and treatment strategies of asthma for primary medical institutions and general practitioners,which refers to the guidelines of asthma diagnosis and treatment published in China and abroad in recent years.It is hoped that it will help to improve the clinical management level of asthma in primary medical institutions,then improve the control level of children’s bronchial asthma and reduce the incidence as well.
