临床药师参与儿童血液内科感染的药学会诊实践与分析

摘　　要：目的:探讨我院血液内科感染药学会诊的特点,为临床药师参与药学会诊及促进临床合理用药提供参考。方法:回顾性分析我院临床药师参与会诊的127例血液内科感染患儿的一般资料、病原菌培养、会诊意见及疾病转归情况。结果:临床药师对127例患儿共会诊166例次,其中6例次与药品不良反应相关,160例次与抗感染相关。127例患儿共检出96株病原菌。会诊意见被全部采纳137例次(82.53%),部分采纳21例次(12.65%),未采纳8例次(4.82%)。会诊意见被全部采纳和部分采纳的患儿中,疾病转归为有效的137例次(86.71%)。采纳情况与疾病转归情况进行双向有序线性趋势检验,线性回归分量有统计学意义(P<0.01),表明预后随着采纳率增加而改善。结论:临床药师参与儿童血液内科感染会诊,为制定合理用药方案提供建议,可促进合理用药,改善临床治疗效果。Objective:To probe into the characteristics of pharmaceutical consultation for children with hematological infection by clinical pharmacists in department of hematology,so as to provide reference for clinical pharmacists to participate into the pharmaceutical consultation and promote the rational drug use in clinic.Methods:General data,bacterial culture,consultation opinion and disease outcome of 127 children with hematological infection consulted by clinical pharmacists in our hospital were retrospectively analyzed.Results:A total of 166 consultations were performed by clinical pharmacists on 127 children,of which 6 were related to adverse drug reactions and 160 were related to anti-infection.Ninety-six strains of pathogenic bacteria were detected in 127 children.The consultation opinion was accepted in 137 cases(82.53%),partially accepted in 21 cases(12.65%),and not accepted in 8 cases(4.82%).Among the children whose consultation opinions were fully or partially accepted,137 cases(86.71%)of the disease outcomes were effective.Two-way ordered linear trend test was performed on adoption and disease outcomes,and the linear regression component was statistically significant(P<0.01),indicating that the prognosis improved with the increase in adoption rate.Conclusion:Clinical pharmacists participate into the consultation of children with hematology infection and provide opinions for formulating rational drug use regimens,which can promote rational drug use and improve clinical treatment effects.
