摘　　要：目的探讨肢体不同病理亚型脂肪肉瘤的超声学表现,以期提高超声检查对脂肪肉瘤临床诊断价值,为脂肪肉瘤的诊断及治疗提供更为可靠的依据。方法选取在本院就诊,并经手术病理证实不同病理亚型脂肪肉瘤患者31例,术前均行超声检查。结果31例肢体脂肪肉瘤中,高分化脂肪肉瘤8例(25.8%),黏液样脂肪肉瘤14例(45.2%),去分化脂肪肉瘤4例(12.9%)和多形性脂肪肉瘤5例(16.1%)。与其他亚型相比,高分化脂肪肉瘤的超声表现通常为内部呈等回声,可见细线状分隔和病灶血供不明显或较少。黏液样脂肪肉瘤的超声表现通常为内部多呈低回声,可见无回声区和病灶血供较丰富。去分化脂肪肉瘤的超声表现通常为典型的高回声和低回声两相区域,两种回声之间分界清,并且大部分病灶血供丰富。多形性脂肪肉瘤的超声表现通常是典型的高回声和低回声形成的类脑回型的混合回声,病灶血供较丰富。结论肢体不同病理亚型脂肪肉瘤超声学表现具有一定的特征性,对脂肪肉瘤的临床诊断及治疗具有一定价值。Objective To investigate the ultrasonographic features of limbs and trunk liposarcoma of different pathologic subtypes in order to improve the value of ultrasound in the clinical diagnosis of liposarcoma and to provide a more reliable basis for the diagnosis and treatment of liposarcoma.Methods 31 patients with liposarcoma of different pathologic subtypes were collectted from Qingdao Municipal Hospital,affiliated to qingdao university,and confirmed by surgery and pathology,all patients underwent ultrasound before surgery.Results In 31 cases of limbs and trunk liposarcoma,8 cases(25.8%)were well differentiated,14 cases(45.2%)were myxoid,4 cases(12.9%)were dedifferentiated,and 5 cases(16.1%)were pleomorphic.Compared to other subtypes,the sonographic appearance of well differentiated liposarcoma was usually isoechoic internally,thin line separations were visible,and inconspicuous or low blood supply to the lesion.The ultrasonographic appearance of myxoid liposarcoma was usually internal hypoechoic,intermingled with anechoic areas and rich blood supply.The sonographic appearance of dedifferentiated liposarcoma was usually specific biphasic pattern of hyperechoic and hypoechoic areas with clear boundaries between the two echoes and rich blood supply in most lesions.The ultrasonographic appearance of pleomorphic liposarcoma was usually typical hyperechoic and hypoechoic gyri-like mixed echogenicity and rich blood supply.Conclusion The ultrasonographic feature of limbs and trunk liposarcoma of different pathologic subtypes have certain characteristics,which are of great value for the clinical diagnosis and treatment of liposarcoma.
