胰岛素微注泵静推在糖尿病合并非霍奇金淋巴瘤患者化疗期间的应用效果

摘　　要：目的探究在糖尿病合并非霍奇金淋巴瘤患者接受化疗治疗期间将胰岛素微注泵静推方法进行应用的临床效果。方法选取2019年2月—2021年9月该院接收的患者共90例,均为糖尿病合并非霍奇金淋巴瘤患者,随机分为对照组和研究组,各45例。对照组多次皮下胰岛素注射,研究组给予胰岛素微注泵静脉推注控制血糖,对两组患者干预后的各项血糖水平变化、血糖达标时间、胰岛素用量和低血糖发生率进行比较。结果治疗前两组患者的各项血糖指标血均较高,差异无统计学意义(P>0.05);治疗后研究组患者的空腹血糖、餐后2 h血糖和糖化血红蛋白分别为(6.34±1.33)mmol/L、(7.90±1.65)mmol/L和(5.98±1.77)%,均比对照组低,差异有统计学意义(t=6.726、2.511、5.804,P<0.05)。研究组的胰岛素使用量和血糖达标所用时间分别为(37.65±6.33)U和(6.54±2.23)d,均优于对照组,差异有统计学意义(t=4.140、6.846,P<0.05)。研究组患者的低血糖发生率为4.44%,低于对照组,差异有统计学意义(χ^(2)=5.075,P<0.05)。结论将胰岛素微注泵推注法应用在糖尿病合并非霍奇金淋巴瘤患者化疗过程中,其临床效果显著,可降低患者的不良反应发生率,促进患者血糖恢复,保证患者体征平稳,应用价值较高。Objective To explore the clinical effect of intravenous injection of insulin microinjection pump in patients with diabetes and non-Hodgkin's lymphoma receiving chemotherapy.Methods A total of 90 patients admitted to the hospital from February 2019 to September 2021 were selected and randomly divided into control group and study group,with 45 patients in each group.The control group was given multiple subcutaneous insulin injections,and the observation group was given intravenous infusion of insulin microinjection pump.The changes of blood glucose levels,the time of blood glucose reaching the standard,the amount of insulin and the incidence of hypoglycemia were compared between the two groups after intervention.Results The blood glucose indexes of patients in the two groups were higher before treatment,and there was no statistically significant difference between the groups(P>0.05).After treatment,fasting blood glucose,2 h postpranpranal blood glucose and hba1c values in the study group were(6.34±1.33)mmol/L,(7.90±1.65)mmol/L and(5.98±1.77)%,respectively,all be lower than those in the control group,and the differences were statistically significant(t=6.726,2.511,5.804,P<0.05).The amount of insulin used in the study group and the time of blood glucose reaching the standard were(37.65±6.33)U and(6.54±2.23)d,respectively,better than the control group,and the difference was statistically significant(t=4.140,6.846,P<0.05).The incidence of hypoglycemia in the study group was 4.44%,lower than that in the control group,and the difference was statistically significant(χ^(2)=5.075,P<0.05).Conclusion The application of insulin microinjection pump bolus method in the chemotherapy of diabetic patients with non-Hodgkin's lymphoma has a significant clinical effect.It can reduce the incidence of adverse reactions in patients,promote the recovery of blood glucose in patients,ensure stable signs of patients,and has high application value.
