经口圈套器牵引辅助下内镜黏膜下剥离术治疗早期胃癌患者的临床疗效及对炎性因子和预后的影响

摘　　要：目的探讨经口圈套器牵引辅助下内镜黏膜下剥离术(ESD)治疗早期胃癌(EGC)患者的临床疗效及对炎性因子和预后的影响。方法根据治疗方法的不同将106例EGC患者分为对照组(n=52,给予传统ESD治疗)和研究组(n=54,给予经口圈套器牵引辅助下ESD治疗)。比较两组患者的临床疗效、炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-6、IL-8、IL-12]水平、并发症发生率及1年复发率。结果研究组患者的总有效率为94.44%,高于对照组患者的80.77%,差异有统计学意义(P﹤0.05)。术后,两组患者的TNF-α、IL-6、IL-8及IL-12水平均较本组术前降低,且研究组患者的TNF-α、IL-6、IL-8及IL-12水平均低于对照组,差异均有统计学意义(P﹤0.05)。研究组患者的并发症总发生率为3.70%,低于对照组患者的17.31%(P﹤0.05)。两组患者的1年复发率比较,差异无统计学意义(P﹥0.05)。结论经口圈套器牵引辅助下ESD治疗EGC患者可提高临床疗效,降低炎性反应和并发症发生率,有助于患者术后康复。Objective To investigate the clinical efficacy of peroral snare traction-assisted endoscopic submucosal dissection(ESD)in patients with early gastric cancer(EGC)and the influence on inflammatory factors and prognosis.Method A total of 106 EGC patients were divided into control group(n=52,given traditional ESD therapy)and study group(n=54,given ESD therapy assisted by peroral snare traction)according to different treatment method.The clinical efficacy,inflammatory factors[tumor necrosis factorα(TNF-α),interleukin(IL)-6,IL-8 and IL-12],complication rate and 1-year recurrence rate were compared between the two groups.Result The total effective rate of the study group was 94.44%,which was significantly higher than 80.77%of the control group(P<0.05).After operation,the levels of TNF-α,IL-6,IL-8 and IL-12 in the two groups were lower than those before operation,with lower levels in the study group than those in the control group(P<0.05).The total incidence of complications in the study group was 3.70%,which was significantly lower than 17.31%in the control group(P<0.05).There was no significant difference in the 1-year re-currence rate between the two groups(P>0.05).Conclusion Peroral snare traction-assisted ESD can improve the clinical efficacy in the treatment of EGC patients,reduce the inflammatory reaction and complications,which is conducive to the postoperative recovery of patients.
