摘 要:目的探讨磁共振动态增强成像(DCE-MRI)定量参数与子宫内膜癌病灶浸润深度的关系。方法选取84例子宫内膜癌患者,均接受DCE-MRI检查,测量其定量参数[容量转移常数(K^(trans))、速率常数(Kep)、血管外细胞外间隙容积比(Ve)]。依据病理学检查病灶肌层浸润深度情况,将84例患者分为未浸润组(n=18)、浅肌层浸润组(n=37)和深肌层浸润组(n=29),比较3组患者的DCE-MRI定量参数。子宫内膜癌浸润深度与DCE-MRI定量参数的相关性采用Pearson相关分析;绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),评估DCEMRI对子宫内膜癌患者肌层浸润的诊断价值。结果3组患者Kep、K^(trans)、Ve比较,差异均有统计学意义(P﹤0.01),浅肌层浸润组、深肌层浸润组患者的Kep、K^(trans)、Ve均高于未浸润组,深肌层浸润组患者的Kep、K^(trans)、Ve均高于浅肌层浸润组,差异均有统计学意义(P﹤0.05)。子宫内膜癌肌层浸润深度与Kep、K^(trans)均呈正相关(r=0.674、0.628,P﹤0.01),与Ve无相关性(r=0.327,P﹥0.05)。K^(trans)、Kep诊断子宫内膜癌患者肌层浸润的AUC分别为0.834、0.806,均大于Ve诊断子宫内膜癌患者肌层浸润的AUC(P﹤0.05)。结论随着子宫内膜癌浸润深度的增加,DCE-MRI定量参数中Kep、K^(trans)、Ve随之增加,Kep、K^(trans)与子宫内膜癌肌层浸润深度呈正相关,且对子宫内膜癌肌层浸润的诊断价值较高。Objective To investigate the relationship between quantitative parameters of dynamic contrast enhanced magnetic resonance imaging(DCE-MRI)and the depth of invasion of endometrial cancer.Method A total of 84 patients with endometrial cancer were selected,and all of them underwent DCE-MRI to measure their quantitative parameters[volume transfer constant(K^(trans)),rate constant(Kep),extracellular space volume ratio(Ve)].According to the pathological examination of the depth of myometrial invasion,84 patients were divided into non-invasive group(n=18),superficial myometrial invasion group(n=37)and deep myometrial invasion group(n=29),and the DCE-MRI quantitative parameters of three groups of patients were compared.The correlation between the depth of invasion of endometrial cancer and quantitative parameters of DCE-MRI was analyzed by Pearson correlation.The receiver operating characteristic(ROC)curve was drawn,and the area under the curve(AUC)was calculated to evaluate the diagnostic value of DCE-MRI for myometrial invasion in patients with endometrial cancer.Result There were significant differences in Kep,K^(trans),and Ve among the three groups(P<0.01).The Kep,K^(trans) and Ve of the patients in the superficial myometrial invasion group and deep myometrial invasion group were significantly higher than those in the non-invasive group(P<0.05),the Kep,K^(trans),and Ve of the deep myometrial invasion group were significantly higher than those of the superficial myometrial invasion group(P<0.05).The depth of myometrial invasion in endometrial carcinoma was positively correlated with Kep and K^(trans)(r=0.674,0.628,P<0.05),but had no correlation with Ve(r=0.327,P>0.05).The AUC of K^(trans) and Kep for the diagnosis of myometrial invasion in patients with endometrial cancer were 0.834 and 0.806,respectively,which were higher than the AUC for the diagnosis of myometrial invasion in patients with endometrial cancer by Ve(P<0.05).Conclusion With the increase of invasion depth of endometrial cancer,Kep,K^(trans) and Ve in q
