摘 要:目的研究基于16S rRNA测序技术分析HLAP患者肠道微生态变化及与IL-1、TNF-α的关联性分析。方法选取2020年6月至2020年12月泉州市第一医院收治的60例急性胰腺炎患者作为病例组,将其中33例高脂血症性急性胰腺炎列入高脂血症性急性胰腺炎(HLAP)组,27例非高脂血症性急性胰腺炎列入非HLAP组,患者均接受益生菌治疗。同时选取61例同期健康体检者作为对照组。对比三组入选者肠道微生态以及血清IL-1、TNF-α水平;对比HALP组和非HLAP组患者肠道微生态丰度。结果与对照组相比,HLAP组和非HLAP组运算分类单位、chao指数和shannon指数显著更低;与非HLAP组相比,HLAP组运算分类单位、chao指数和shannon指数显著更低(P<0.05);与对照组相比,HLAP组和非HLAP组IL-1和TNF-α水平显著更高;与非HLAP组相比,HLAP组IL-1和TNF-α水平显著更高(P<0.05);相比于非HLAP组患者,HLAP组患者厚壁菌门、拟杆菌门显著更低,变形菌门、梭杆菌门以及放线菌门显著更高(P<0.05);相比于非HLAP组患者,HLAP组患者双歧杆菌属、链球菌属、萨特菌属和艾克曼菌属显著更高,拟杆菌属、瘤胃球菌、梭菌属、戴阿利斯特菌属以及巨球菌属显著更低(P<0.05)。结论HLAP患者存在较为明显的肠道微生态失调,同时患者血清IL-1、TNF-α水平也存在明显异常,与肠道微生态变化存在相关性。Objective To study the analysis of intestinal microecological changes in HLAP patients and their correlation with IL-1 and TNF-αbased on 16S rRNA sequencing technology.Methods From June 2020 to December 2020,60 patients with acute pancreatitis of Quanzhou First Hospital were selected as the pathology group,of which 33 cases of hyperlipidemia acute pancreatitis were included in the hyperlipidemia acute pancreatitis(HLAP)group and 27 cases of non-hyperlipidemia acute pancreatitis were included in the non-HLAP group.All patients received probiotic treatment.At the same time,61 cases of healthy physical examination during the same period were selected as the control group.The intestinal microbiota and serum IL-1 and TNF-αof the three groups and the intestinal microecological abundance of patients in the HALP group and the non-HLAP group were compared.Results Compared with the control group,the operational taxon,chao index,and shannon index of the HLAP group and the non-HLAP group were significantly lower;compared with the non-HLAP group,the operational taxon,chao index,and shannon index of the HLAP group were significantly lower(P<0.05).Compared with the control group,the levels of IL-1 and TNF-αin the HLAP group and the non-HLAP group were significantly higher;compared with the non-HLAP group,the levels of IL-1 and TNF-αwere significantly higher in the HLAP group(P<0.05).Compared with non-HLAP group patients,Firmicutes and Bacteroides in HLAP group were significantly lower,whereas Proteobacteria,Fusobacteria and Actinomycetes were significantly higher(P<0.05);compared with the non-HLAP group,Bifidobacterium,Streptococcus,Sartorella,and Ekmania were significantly higher in patients in the HLAP group,while Bacteroides,Rumenococcus,Clostridium,Diallisteria,and Macrococcus were significantly lower(P<0.05).Conclusion Patients with HLAP have obvious intestinal microecological disorders.At the same time,there are also obvious abnormalities in serum IL-1 and TNF-α,which are related to changes in intestinal microecologi
