摘 要:目的探讨早期宫颈癌患者宫颈注射纳米炭前哨淋巴结(SLN)检出情况,并分析其相关影响因素。方法选择经宫颈刮片检查确诊的127例早期宫颈癌患者作为研究对象,全部患者均经宫颈注射纳米炭行SLN示踪检查,分析SLN的检出情况。根据检出情况将患者分为检出组与未检出组,设计基线资料调查表,比较2组患者基线资料,分析影响早期宫颈癌患者SLN检出的因素。结果127例早期宫颈癌患者宫颈注射纳米炭SLN检出99例,检出率为77.95%(99/127)。未检出组接受过相关治疗、存在淋巴结局部肿大、选择4点宫颈部位注射的人数占比高于检出组,病灶直径大于检出组,差异有统计学意义(P<0.05);组间其他资料比较差异无统计学意义(P>0.05);Logistic回归分析结果显示,接受过相关治疗、存在局部淋巴结肿大、病灶直径较大、宫颈注射部位为4点是早期宫颈癌患者经宫颈注射纳米炭SLN未检出的影响因素(OR>1,P<0.05)。结论早期宫颈癌患者经宫颈注射纳米炭SLN检出率较高,检出情况可能受接受过相关治疗、局部淋巴结肿大、病灶直径及宫颈注射部位的影响。Objective To investigate the detection of sentinel lymph nodes(SLN)injected with nano carbon in patients with early cervical cancer,and to analyze its related influencing factors.Methods 127 patients with early cervical cancer diagnosed by cervical scraping were selected as the research subjects,all patients underwent SLN tracing examination by cervical injection of nano carbon,the detection of SLN by cervical injection of nano carbon was analyzed,according to the detection situation,the patients were divided into detection group and non detection group;designed baseline data questionnaire,the baseline data of patients in the two groups were compared,the influencing factors of SLN detection in patients with early cervical cancer were analyzed.Results Among 127 patients with early cervical cancer,99 cases were detected by cervical injection of nano carbon SLN,and the detection rate was 77.95%(99/127).The proportions of patients who had received relevant treatment,had lymphadenopathy and chose cervical injection at 4 points in the non detection group were higher than those in the detection group,and the lesion diameter was larger than that in the detection group,with statistical significant differences(P<0.05);there was no statistical significant difference in comparison of other data between groups(P>0.05);through Logistic regression analysis,the results showed that receiving relevant treatment,local lymphadenopathy,large lesion diameter and 4 points of cervical injection site wee the influencing factors that were not detected by cervical injection of nano carbon SLN in patients with early cervical cancer(OR>1,P<0.05).Conclusion The detection rate of nano carbon SLN injected through cervix in patients with early cervical cancer is high,the detection may be affected by relevant treatment,local lymphadenopathy,lesion diameter and cervical injection site.
