摘　　要：目的观察计算机辅助设计制作(computer aided design/computer aided manufacture,CAD/CAM)的一体化纤维桩核结合原有全冠修复折断前牙的临床效果。方法选取前牙全冠修复后基牙折断病例14例,其中上前牙13例,下前牙1例,采用CAD/CAM一体化纤维桩核结合原全冠进行重建修复。临床效果评价采用改良美国公共健康协会标准(USPHS),在修复后即刻及修复后6、12、24、36个月检查,观察指标包括基牙和桩核冠的完整性,冠边缘密合度、继发龋与冠边缘着色情况,对入组病例进行Kaplan-Meier生存分析。结果所有患者在36个月的观察期内均未发生基牙或桩核冠折裂,无冠边缘继发龋发生,桩核冠存留率100%。修复后第12个月随访出现冠边缘着色1例,修复后第24个月随访出现冠边缘密合度下降并着色1例,修复后并发症发生率18.2%。结论CAD/CAM一体化纤维桩核结合原有全冠修复前牙基牙折断的临床效果良好,是修复此类牙体缺损较好的选择。Objective To evaluate the clinical effect of CAD/CAM one-piece fiber post-and-core combined with the original full crown to restore the fractured anterior teeth crown.Methods Forteen fractured anterior teeth were recruited.The fractured anterior teeth were restored with CAD/CAM customized one-piece fiber post-and-core combined with the original metal ceramic or all ceramic crown.The clinical effect was evaluated at the time point of immediate,6 months,12 months,24 months,and 36 months after restoration.The performance on the integrity of the restoration complex,the marginal adaptation,secondary caries and the marginal discoloration after restoration were recorded,and the Kaplan-Meier survive curve was derived from the results.Results Among the 14 teeth,no restoration and abutment fracture or marginal secondary caries occurred,the survival rate of the restoration was 100%.The complication rate was 18.2%while one case of marginal discoloration was detected at the 12 months follow up,and another case of marginal adaptation degeneration accompanied by marginal discoloration was found at the 24th month follow up.Conclusions The use of customized one-piece fiber post-and-core combined with the original crown can be used to restore the fractured anterior teeth crown.
