口腔癌患者前臂或股前外侧游离皮瓣修复成功术后D-二聚体水平短期变化规律的研究

摘　　要：目的检测接受前臂或股前外侧皮瓣修复成功的口腔癌患者术后近期血浆D-二聚体检测值,分析其变化规律及相关因素。方法本研究纳入接受前臂或股前外侧皮瓣修复成功的口腔癌患者共94例,检测术前和术后1、2、3天D-二聚体值,使用重复测量方差分析分析其组间差异和相关因素。结果本组病例术前D-二聚体水平均正常,术后第1、2、3天D-二聚体平均值分别为3.93、1.54、1.98 mg/L。年龄>60岁与≤60岁者、接受单侧或双侧颈淋巴清扫者之间D-二聚体值有显著差异。结论大多数口腔癌患者接受扩大切除、皮瓣修复术后,D-二聚体水平升高。术后血浆D-二聚体接近本研究平均值的患者可能在皮瓣危象风险的安全范围内,术后第1天与第3天时应提高皮瓣出现静脉危象的警惕性。年龄大于60岁是D-二聚体升高的危险因素。Objective D-dimer plasma levels in patients receiving successful reconstruction with forearm or ALT flap were examined.Methods The oral cancer patients receiving successful repair with forearm or ALT free flap were included in this study.D-dimer plasma levels were examined before and 1-3 days after operation.Results The D-dimer level in all patients before operation was in normal range.The average of D-dimer level was 3.93,1.54,and 1.98 mg/L on day 1,2 and 3 after operation.D-dimer levels in patients aged over 60 years was higher than those of patients under 60 years old.D-dimer level was significantly different between patients who received unilateral or bilateral cervical node dissection.Conclusions D-dimer levels in patients after tumor resection and free flap reconstruction increased.The patients over 60-years-old had higher risk of elevated D-dimer level.
