盐酸肾上腺素注射液在翼状胬肉术中的应用

摘　　要：目的观察盐酸肾上腺素注射液滴眼在翼状胬肉术中的止血效果及对心率、血压和瞳孔大小的影响。方法选取2019年7月至2021年12月在长治医学院附属和平医院就诊治疗的60例(60眼)原发性翼状胬肉患者,按随机数字表法分为试验组、对照组,各30例(30眼)。试验组于手术开始前10 min及手术开始时术眼分别滴盐酸肾上腺素注射液1滴。对照组不滴用血管收缩药物。观察比较两组患者手术不同时间的结膜充血情况、血压、心率、瞳孔大小、术野清晰度等。结果试验组患者手术中结膜充血评分低于对照组(P<0.05)。试验组和对照组在手术前、手术开始时、手术10 min及手术毕的各时间点的血压、心率比较,差异均无统计学意义(P>0.05)。而试验组自手术开始至手术结束时瞳孔逐渐散大,与对照组比较差异具有统计学意义(P<0.05)。试验组需要烧灼止血的患者例数少于对照组,手术视野清晰度高于对照组,且手术时间与对照组相比缩短(P<0.05)。结论翼状胬肉术中应用盐酸肾上腺素注射液滴眼能有效减少出血,提高手术效率和效果,且对心率和血压无明显影响,但可使瞳孔明显散大。Objective To observe the hemostatic effect of adrenaline hydrochloride injection in pterygium surgery and its effects on heart rate,blood pressure and pupil size.Methods A total of 60 patients(60 eyes)with primary pterygium who were treated in Heping Hospital Affiliated to Changzhi Medical College from July 2019 to December 2021 were selected and divided into the experimental group and the control group by random number table method,with 30 cases(30 eyes)in each.In the experimental group,one drop of adrenaline hydrochloride injection was dropped 10 minutes before and at the beginning of the operation respectively.The control group did not use any vasoconstrictor drugs.The conjunctival congestion,blood pressure,heart rate,pupil size and clarity of operation field at different times were observed and compared between the two groups.Results The conjunctival congestion score in the experimental group was lower than that in the control group(P<0.05).There was no statistical difference in blood pressure and heart rate between the experimental group and the control group at each time point:before operation,at the beginning of operation,10 minutes after operation and after operation(P>0.05).The pupils of the experimental group gradually dilated from the beginning of the operation to the end of the operation,which was statistically significant compared with the control group(P<0.05).The number of patients requiring cautery for hemostasis in the experimental group was less than that in the control group,the clarity of surgical field was higher than that in the control group,and the operation time was shorter compared with the control group(P<0.05).Conclusion The application of adrenaline hydrochloride injection eye drops in pterygium surgery can effectively reduce bleeding,improve operation efficiency and effect,and has no significant effect on heart rate and blood pressure,but it can make pupils dilate significantly.
