脉管复康胶囊辅助阿托伐他汀钙片、阿司匹林治疗脑动脉硬化症患者的效果

摘　　要：目的探讨脉管复康胶囊辅助阿托伐他汀钙片、阿司匹林治疗脑动脉硬化症(CAS)的效果。方法将2020年8月至2021年7月于漯河市第六人民医院就诊的96例CAS患者随机分为联合组(n=48)、对照组(n=48)。对照组接受阿托伐他汀钙片、阿司匹林治疗,联合组在对照组基础上接受脉管复康胶囊治疗,对比两组临床疗效、血脂水平[甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)]、颈部B超[双侧颈动脉内层中膜厚度(IMT)、颈动脉斑块面积]、脑血流动力学[血管搏动指数(PI)、血管阻力指数(RI)、收缩期血流速度(V_(s))、舒张期血流速度(V_(d))]及炎症因子[肿瘤坏死因子-α(TNF-α)、基质金属蛋白酶(MMP9)、白细胞介素-1(IL-1)]。结果联合组治疗总有效率(95.83%)高于对照组(83.33%)(P<0.05);治疗3个月后,两组TG、TC、LDL水平均降低,且联合组各指标水平低于对照组(P<0.05);治疗3个月后,联合组颈动脉斑块面积较对照组降低(P<0.05);治疗3个月后,两组PI、RI水平均降低,V_(s)、V_(d)水平均升高,且联合组各指标改善程度优于对照组(P<0.05);治疗3个月后,联合组与对照组TNF-α、MMP9、IL-1水平低于对照组(P<0.05)。结论脉管复康胶囊辅助阿托伐他汀钙片、阿司匹林治疗CAS,可有效改善血脂水平、脑部血流动力学,减轻炎症反应,缩小颈动脉斑块面积,疗效显著。Objective To investigate the efficacy of Maiguanfukang capsules combined with atorvastatin calcium tablets and aspirin in the treatment of cerebral arteriosclerosis(CAS).Methods A total of 96 CAS patients who visited the Sixth People’s Hospital of Luohe City from August 2020 to July 2021 were randomly divided into the combination group(n=48)and the control group(n=48).The control group was treated with atorvastatin calcium tablets and aspirin,and the combination group was treated with Maiguanfukang capsules on the basis of the control group.The clinical efficacy,blood lipid levels[triglyceride(TG),total cholesterol(TC),high-density lipoprotein(HDL),low-density lipoprotein(LDL)],neck B-ultrasound[bilateral carotid intima-media thickness(IMT),carotid plaque area],cerebral hemodynamics[vascular pulsation index(PI),vascular resistance index(RI),systolic blood flow velocity(V_(s)),diastolic blood flow velocity(V_(d))]and inflammatory factors[tumor necrosis factor-α(TNF-α),matrix metalloproteinase(MMP9),interleukin-1(IL-1)]were compared between the two group.Results The total effective rate of treatment in the combination group was 95.83%,which was higher than that in the control group(83.33%)(P<0.05).After 3 months of treatment,the carotid plaque area in the combination group was lower than that in the control group(P<0.05).After 3 months of treatment,the levels of PI and RI in the two groups were decreased,and the levels of V_(s) and V_(d) were the increased,and the improvement degree of indexes in combination group was better than that in control group(P<0.05).After 3 months of treatment,the levels of TNF-α,MMP9,and IL-1 in the combination group were decreased compared with those in the control group(P<0.05).Conclusion Maiguanfukang capsules combined with atorvastatin calcium tablets and aspirin in the treatment of CAS can effectively improve blood lipid levels,cerebral hemodynamics,reduce inflammatory response,and reduce the area of carotid plaque,with significant curative effect.
