摘  要:目的探讨造血祖细胞激酶-1(HPK1)、磷脂酰肌醇3-激酶(PI3K)与乳腺癌根治术患者5年预后结局的关系。方法选取2014年1月至2016年12月在琼海市人民医院经乳腺癌根治手术治疗的144例患者进行回顾性分析,所有患者手术后均对病灶组织进行免疫组织化学染色检测,根据患者5年随访结局中是否出现复发转移将其分为转移组33例、非转移组111例。并对比两组患者的HPK1、PI3K蛋白阳性表达率差异,采用生存分析法分析不同HPK1、PI3K蛋白表达的乳腺癌患者无进展生存期差异,并采用Logistic回归模型分析HPK1、PI3K蛋白与患者预后的关系。结果随访5年,144例乳腺癌患者,出现术后肿瘤转移26例、术后复发7例,全部纳入转移组。转移组病灶组织中的HPK1蛋白阳性表达率为24.24%,显著低于非转移组患者(60.36%),PI3K蛋白阳性表达率为66.67%,显著高于非转移组患者(46.85%),差异均有统计学意义(P<0.05)。HPK1蛋白阳性表达患者的无进展生存时间长于HPK1蛋白阴性表达患者,差异有统计学意义(P<0.05);PI3K蛋白阳性表达患者的无进展生存时间与PI3K蛋白阴性表达患者比较,差异无统计学意义(P>0.05)。Logistic回归模型结果显示,临床分期越高、发生淋巴结转移、发生脉管侵犯是乳腺癌根治术后发生复发转移的独立危险因素(P<0.05),HPK1蛋白阳性表达可降低乳腺癌根治术后发生复发转移的风险(P<0.05)。结论HPK1蛋白表达下调与乳腺癌患者根治术后转移复发具有密切联系,PI3K蛋白表达变化与乳腺癌根治术后患者转移复发的关系尚需要进一步研究。Objective To investigate the relationship between hematopoietic progenitor kinase 1(HPK1),phosphatidylinositol 3-kinase(PI3K)and 5-year prognosis of breast cancer patients undergoing radical mastectomy.Methods A total of 144 cases of breast cancer patients who underwent radical surgery for breast cancer from January,2014 to December,2016 in Qionghai People's Hospital were selected for retrospective analysis.All patients underwent immunohistochemical staining on the lesion tissue after surgery.According to the 5-year follow-up of patients whether recurrence and metastasis occurred in the outcome were divided into 33 cases in the metastasis group and 111 cases in the non-metastasis group,and the differences in the positive expression rates of HPK1 and PI3K proteins were compared between the two groups.Survival analysis was used to analyze the different HPK1 and PI3K proteins.The expression of breast cancer patients was different in progression-free survival,and logistic regression model was used to analyze the relationship between HPK1 and PI3K protein and the prognosis of patients.Results Followed up for 5 years,144 breast cancer patients,26 cases of postoperative tumor metastasis and 7 cases of postoperative recurrence,were all included in the metastasis group.The positive expression rate of HPK1 protein in the lesions of the metastasis group was 24.24%,which was significantly lower than that of the non-metastasis group(60.36%),the positive expression rate of PI3K protein in the metastatic group was 66.67%,which was significantly higher than that in the non-metastatic group(46.85%),and the differences were statistically significant(P<0.05).The progression-free survival time of patients with positive expression of PI3K protein was not significantly different from that of patients with negative expression of PI3K protein(P>0.05).Logistic regression model results showed that higher clinical stage,lymph node metastasis and vascular invasion were independent risk factors for recurrence and metastasis after radical mas
