高脂血症性急性胰腺炎患者血清血脂水平与凝血功能相关性分析

摘　　要：目的探讨高脂血症性急性胰腺炎(HLAP)患者血脂与凝血指标水平的变化,同时分析患者血清中两种指标水平间的相关性。方法选择贵州航天医院2018年6月—2021年6月收治的126例HLAP患者为研究对象,根据亚特兰大急性胰腺炎严重程度分级标准分为轻症组患者51例,中-重症组患者42例,重症组患者33例,检测3组患者血清中甘油三酯(TG)、凝血酶原时间(PT)及活化部分凝血活酶时间(APTT)水平,比较3组患者间TG、PT、APTT三种指标的差异,同时分析上述指标间的相关性及,采用受试者工作特征曲线(ROC)分析患者血清PT、APTT、TG水平对HLAP严重程度的诊断价值。结果3组患者血清中TG、PT、APTT水平差异显著(P<0.05),且重症组患者血清TG、PT、APTT水平显著高于中-重症组和轻症组患者,中-重症组患者血清TG、PT、APTT水平显著高于轻症组,差异均有统计学意义(P<0.05)。经Pearson相关性分析结果显示,患者血清TG表达水平与PT、APTT表达水平间均呈显著正相关(P<0.05),血清PT、APTT、TG表达水平诊断HLAP严重程度的ROC曲线下面积分别为0.825、0.742、0.699,而联合检测诊断的ROC曲线下面积为0.897,显著高于单项指标,差异有统计学意义(P<0.05)。结论HLAP患者血清TG、PT、APTT水平与病情严重程度相关,且患者TG与PT、APTT水平间均呈显著正相关,综合考虑上述指标有助于预测HLAP患者的严重程度。Objective To explore the changes of lipid level and coagulation in patients with hyperlipidemic acute pancreatitis(HLAP)and analyze the correlation between lipid level and coagulation.Methods This study enrolled 126 patients with HLAP in Guizhou aerospace hospital from June 2018 to June 2021 and 51 patients were enrolled into mild group,42 patients were enrolled into moderate severe group and 33 patients were enrolled into severe group according to the severity classification standard.The serum triglyceride(TG),prothrombin time(PT)and activated partial thromboplastin time of the three groups were measured(APTT).The differences of TG,PT and APTT among the three groups were compared and analyzed the correlations between the above indexes.The receiver operating characteristic curve(ROC)were analyzed to analyze the diagnostic value of serum PT,APTT and TG levels in the severity of HLAP.Results The levels of serum TG,PT and APTT in the three groups were significantly different(P<0.05),and the expression levels of serum TG,PT and APTT in the severe group were significantly higher than those in the moderate severe group and mild disease group(P<0.05).The expression levels of serum TG,PT and APTT in the moderate severe group were significantly higher than those in the mild disease group(P<0.05).The results of Pearson correlation analysis showed that there was a significant positive correlation between the expression level of serum TG and the expression level of PT and APTT in all patients(P<0.05).The areas under the ROC curve of serum PT,APTT and TG in the diagnosis of hlap severity were 0.825,0.742 and 0.699 respectively,while the area under the ROC curve of combined detection was 0.897,which was significantly higher than that of single index(P<0.05).Conclusion The levels of serum TG,PT and APTT in patients with HLAP are correlated with the severity of the disease,and there is a significant positive correlation between TG,PT and APTT.Comprehensive consideration of the above indicators is helpful to predict the severity
