摘 要:目的 探讨钙蛋白酶小亚基1(CAPN4)、程序性死亡配体1(PD-L1)和骨桥蛋白(OPN)在多中心性乳腺癌组织中的表达及意义。方法 选取2020年1月至2021年2月间上海市崇明区第三人民医院收治的71例多中心性乳腺癌患者,收集患者的癌组织及癌旁组织,比较两者蛋白表达情况,分析CAPN4、PD-L1和OPN与多中心性乳腺癌病理特征的关系,分析患者预后的影响因素。结果 CAPN4阳性染色表达主要集中在细胞质,呈淡黄色至棕黄色;PD-L1蛋白阳性表达主要集中在细胞浆及细胞膜,呈棕黄色;OPN蛋白阳性表达主要集中在细胞质,呈棕黄色及黄色颗粒。多中心性乳腺癌组织的CAPN4、PD-L1和OPN蛋白表达均高于癌旁组织,差异均有统计学意义(均P<0.05)。CAPN4、PD-L1和OPN与患者年龄、淋巴转移和分子分型无相关性(P>0.05),与患者组织学分级、肿瘤直径、组织学类型和Ki-67增殖指数有相关性(均P<0.05)。以多中心性乳腺癌患者预后为因变量,将CAPN4、PD-L1和OPN、组织学分级、肿瘤直径、组织学类型和Ki-67增殖指数作为自变量,多因素Logistics回归分析显示,CAPN4、PD-L1、OPN、组织学分级、肿瘤直径、组织学类型和Ki-67增殖指数是多中心性乳腺癌患者预后的独立影响因素(均P<0.05)。结论 CAPN4、PD-L1和OPN在多中心性乳腺癌患者中呈高表达,且与患者的组织学分级、肿瘤直径、组织学类型及Ki-67增殖指数密切相关,在多中心性乳腺癌患者中具有重要意义。Objective To investigate the expression and significance of calpain small subunit 1(CAPN4), programmed cell death ligand 1(PD-L1) and osteopontin(OPN) in patients with multicentric breast cancer. Methods A total of 71 patients with multi-central breast cancer who were diagnosed and treated at Third People’s Hospital, Chongming District, Shanghai from January 2020 to February 2021 were selected. Cancer tissues and paracancerous tissues were collected. Protein expression of cancer tissues and paracancerous tissues was compared. The relationship between Capn4, PD-L1, OPN and pathological features of multi-central breast cancer was analyzed. Multifactor logistic regression was used to analyze factors influencing the prognosis of patients with multicentral breast cancer. Results CAPN4 positive expression was mainly concentrated in cytoplasm, showing light yellow to brownish yellow color. The positive expression of PD-L1 protein mainly concentrated in the cytoplasm and membrane, showing yellow and brownish yellow color. OPN protein was mainly expressed in cytoplasm and was yellow and brownish yellow. The protein expressions of CAPN4, PD-L1 and OPN in multi-center breast cancer tissues were higher than those in paracancerous tissues(all P<0.05). CAPN4, PD-L1 and OPN were not associated with age, lymphatic metastasis and molecular typing(P>0.05). Instead, they were associated with histological grade, tumor diameter, histological type and Ki-67 proliferation index(all P<0.05). Taking CAPN4, PD-L1, OPN, histological grade, tumor diameter, histological type and Ki-67 proliferation index as independent variables and using a multi-factor logistic regression analysis, the results showed that CAPN4, PD-L1, OPN, histological grade, tumor diameter, histological type and Ki-67 proliferation index were independent factors for prognosis of multicentric breast cancer. Conclusion CAPN4, PD-L1 and OPN are highly expressed in patients with multi-central breast cancer, and are closely associated with histological grade, tumor diameter,
