手辅助腹腔镜治疗胰腺实性假乳头状肿瘤一例

摘　　要：胰腺实性假乳头状肿瘤(SPN)是一类多为单发的罕见低度恶性肿瘤,以手术治疗为主。该文报道了1例应用手辅助腹腔镜技术治疗SPN的诊疗经过,该例为29岁女性患者,因胸骨后疼痛伴憋闷不适3个月余入院,行腹部增强CT提示为脾胰间隙囊性占位,约5.4 cm×3.7 cm,予手辅助腹腔镜下胰尾联合脾切除术,术后病理示SPN,患者术后3 d肛门排气并进食流质,术后1周出院,随访8个月未见肿瘤复发。该病例为我国治疗SPN提供了临床参考。Solid pseudopapillary neoplasms(SPN)of the pancreas are a type of mostly single and rare low-grade malignant tumors,which are mainly treated by surgery.In this article,the diagnosis and treatment of one case of SPN who was treated using handassisted laparoscopy were reported.The 29-year-old woman patient was admitted to the hospital due to retrosternal pain accompanied by suppressed discomfort for more than 3 months.Enhanced abdominal CT revealed cystic space-occupying lesions between the spleen and pancreas(approximately 5.4 cm×3.7 cm).Hand-assisted laparoscopic caudal pancreatic resection combined with splenectomy was performed.The diagnosis of SPN was confirmed by postoperative pathology.The patient had anal exhaust and feeding fluid at 3 days after surgery,and she was discharged at 1 week after surgery.No tumor recurrence was observed during 8 months of follow-up.This case provides clinical reference for the treatment of SPN in China.
