去表皮三角形皮瓣联合“双环法”隧道悬吊缝合矫正乳头内陷的临床研究

摘　　要：目的探讨去表皮三角形皮瓣联合“双环法”隧道悬吊缝合矫正乳头内陷的效果。方法自2017年3月至2019年9月,华中科技大学同济医学院附属湖北妇幼保健院医疗美容科收治的37例(67个乳头)乳头内陷患者,通过去表皮三角形皮瓣联合乳头基底贯通隧道,于乳头颈部横向及纵向垂直方向设计两个的“矩形回路”,缝合收紧乳头颈,悬吊填充乳头基底空虚腔隙,双重收紧矫正乳头内陷。结果37例患者术后切口均一期愈合,乳头及乳晕血运均良好;所有患者获随访6~18个月,乳头功能良好,切口瘢痕不明显,外形满意,无复发。结论该术式切口较小,操作简便,效果确切不易复发,且较少出现并发症,是一种理想的矫正乳头内陷的微创手术方法。Objective To explore the efficacy of deepithelialized triangular flap combined with double round-block tunnel suspension suture in the correction of inverted nipple.Methods During the period of March 2017 to September 2019,37 patients(67 nipples)with mild,moderate or severe inverted nipples were treated in our hospital.The deepithelialized triangular flap combined with the base of the nipple were designed to penetrate through the tunnel.Two"rectangular circuits"were designed in the transverse and vertical directions of the neck of the nipple.The neck of the nipple was sutured and tightened,and the empty cavity of the base of the nipple was suspended and filled.The inverted nipple was corrected by double tightening.Results All incisions in 37 patients healed by first intension.The blood supply of nipple-areola complex was good.All patients were followed up for 6 to 18 months.The function of nipple was good,and the scar in the incision was not obvious.The appearance was satisfactory,and no recurrence was found.Conclusion This is a minimally invasive method with small incision,simple operation and good efficacy.The recurrence and incidence of complications are low.This is an ideal method in the correction of inverted nipple.
