摘 要:目的比较绝经后乳腺癌患者^(99m)Tc-亚甲基二磷酸盐(^(99m)Tc-MDP)骨显像中不同部位的ROI比值,了解是否能够通过骨显像评估此类患者的骨密度(BMD)状况。方法回顾性分析82例接受^(99m)Tc-MDP骨显像及双能X线BMD检测的绝经后乳腺癌患者,根据BMD结果将其分为骨量正常组、骨量减低组和骨质疏松组,分别计算骨显像中腰椎、左侧股骨颈、颅顶骨、左侧胫骨中段与左侧股骨中段的ROI比值,分析腰椎、左侧股骨颈ROI比值与BMD值的相关性,比较不同部位ROI比值在3组间的差异,并比较3组内针对乳腺癌进行的内分泌治疗对不同部位ROI比值的影响。结果左侧股骨颈ROI比值与左侧股骨颈BMD值呈正相关(r=0.276,P<0.05),腰椎ROI比值与腰椎BMD值无明显相关(r=0.173,P>0.05)。3组间骨量减低组的颅顶骨ROI比值高于骨量正常组与骨质疏松组(均为P<0.05),骨质疏松组的左侧股骨颈ROI比值低于骨量正常组及骨量减低组(均为P<0.05),3组间腰椎、左侧胫骨中段ROI比值的差异无统计学意义(P>0.05)。仅有骨量正常组中的内分泌治疗组左侧胫骨中段ROI比值低于无内分泌治疗组(P<0.05)。结论绝经后乳腺癌患者能够通过全身骨显像一次检查同时评估骨转移及骨密度情况,颅顶骨及左侧股骨颈的ROI比值对临床诊断及治疗后随访更有价值。Objective To compare the ROI ratios of different sites in ^(99m)Tc-methylene diphosphonate(^(99m)Tc-MDP)bone scintigraphy(BS)in postmenopausal breast cancer patients to understand whether BS could be used to evaluate bone mineral density(BMD)in the patients.Methods Retrospective analysis was performed on 82 postmenopausal breast cancer patients who received ^(99m)Tc-MDP BS and dual-energy X-ray absorptiometry(DXA)examination.The patients were divided into normal,osteopaenia and osteoporosis groups according to BMD results.The ROI ratios of the lumbar vertebrae,left femoral neck,parietal bone,and left middle segment of the tibia in the BS were calculated,and differences among the three groups were compared.The correlation between ROI ratios and BMD values for lumbar vertebrae and the left femoral neck was analysed.The effects of endocrine therapy for breast cancer treatment on the ROI ratios of different parts in the 3 groups were compared.Results The ROI ratio and the BMD of the left femoral neck correlated significantly positively(r=0.276,P<0.05);those of the lumbar vertebra did not correlate(r=0.173,P>0.05).Among the 3 groups,the ROI ratio of the parietal bone was higher in the osteopaenia group than in the normal and osteoporosis groups,and the ROI ratio of the left femoral neck of the osteoporosis group was lower than those of the other two groups(all P<0.05).Conversely,there was no significant difference in ROI ratios of the lumbar vertebra and left middle tibia among the 3 groups(P>0.05).Only in the normal group,the ROI ratio of the left middle segment of the tibia was higher in the endocrine therapy group than that in the non-endocrine therapy group(P<0.05).Conclusion ^(99m)Tc-MDP BS could be used to evaluate bone metastasis and BMD at the same time for postmenopausal breast cancer patients.The ROI ratios of the parietal bones and left femoral neck were more valuable for clinical diagnosis and post-treatment follow-up.
