卵巢癌患者多种血清肿瘤标志物表达水平及其对疾病的诊断价值研究

摘　　要：目的:探究癌胚抗原(CEA)、糖类抗原199(CA199)、糖类抗原125(CA125)、甲胎蛋白(AFP)在卵巢癌患者中的表达水平及诊断价值。方法:选取92例卵巢良性疾病患者(卵巢良性疾病组),87例卵巢癌患者(卵巢癌组)及95例健康体检者(对照组)为研究对象。比较对照组、卵巢良性疾病组、卵巢癌组血清CEA、CA199、CA125、AFP水平,不同分期卵巢癌患者血清CEA、CA199、CA125、AFP水平,对照组、卵巢良性疾病组、卵巢癌组阳性率及CEA、CA199、CA125、AFP诊断卵巢癌的诊断效能。结果:卵巢癌组血清CEA、CA199、CA125、AFP水平均高于卵巢良性疾病组、对照组,且卵巢良性疾病组血清CA125水平高于对照组(均P<0.05);卵巢癌Ⅲ期患者血清CEA、CA199、CA125、AFP水平均高于Ⅱ期、Ⅰ期患者,且Ⅱ期患者高于Ⅰ期患者(均P<0.05);卵巢癌组CEA、CA199、CA125、AFP单项及联合检测阳性率高于卵巢良性疾病组、对照组,CEA、CA199、CA125、AFP四项联合检测卵巢癌的灵敏度、特异度、阳性预测值、阴性预测值、准确度分别为98.80%、75.00%、98.80%、75.00%、97.70%,均高于各项单独检测(均P<0.05)。结论:卵巢癌患者血清CEA、CA199、CA125、AFP水平高于卵巢良性疾病患者,卵巢癌患者血清CEA、CA199、CA125、AFP水平与临床分期呈正相关,且CEA、CA199、CA125、AFP四项联合检测诊断效能高于单项检验。Objective:To investigate the expression levels and diagnostic value of carcinoembryonic antigen(CEA),carbohydrate antigen 199(CA199),carbohydrate antigen 125(CA125)and alpha-fetoprotein(AFP)in patients with ovarian cancer.Methods:A total of 92 patients with benign ovarian disease(benign ovarian disease group),87 ovarian cancer patients(ovarian cancer group)and 95 healthy patients were selected as the research objects.The serum levels of CEA,CA199,CA125 and AFP of three groups were compared.The serum levels of CEA,CA199,CA125 and AFP levels of ovarian cancer patients with different stages were compared.Positive rate of three groups and diagnostic efficacy of CEA,CA199,CA125,AFP in ovarian cancer were compared.Results:The serum levels of CEA,CA199,CA125 and AFP in ovarian cancer group were higher than those in benign ovarian disease group and the control group,and the serum level of CA125 in benign ovarian disease group was higher than that in control group(all P<0.05).The serum levels of CEA,CA199,CA125 and AFP in stage Ⅲ ovarian cancer patients were higher than those in stage Ⅰ and Ⅱ,and the levels in stage Ⅱ were higher than those in stageⅠ(all P<0.05).The positive rate of CEA,CA199,CA125 and AFP in single and combined tests in ovarian cancer group were higher than those in benign ovarian disease group and control group,and those in benign ovarian disease group were higher than those in control group(all P<0.05).The sensitivity,specificity,positive predictive value,negative predictive value,and accuracy of CEA,CA199,CA125,and AFP in combined test for ovarian cancer were 98.80%,75.00%,and 98.80%,75.00% and 97.70%,which were higher than those in single test(all P<0.05).Conclusion:The serum levels of CEA,CA199,CA125,and AFP in ovarian cancer patients were higher than those in patients with benign ovarian diseases.The levels of serum CEA,CA199,CA125,and AFP in ovarian cancer patients were positively correlated with clinical stage,and the diagnostic efficiency of CEA,CA199,CA125 in combined test and AFP was
