前房注射头孢呋辛对白内障术后眼内炎的预防作用

摘　　要：目的评价白内障手术中前房注射头孢呋辛预防眼内炎的临床、经济影响。方法回顾性研究前房注射头孢呋辛之前(2003年~2009年)及之后(2010年~2015年)白内障术后眼内炎的发病率,通过比较计算相对危险度(RR)来研究头孢呋辛是否具有预防作用,通过计算归因危险分数(AF)研究头孢呋辛对于预防眼内炎的临床影响度,通过计算需治疗人数(NNT)及其成本研究其经济影响。结果共统计了6004例白内障患者手术,其中2003年~2009年3284例,2010年~2015年2720例,其中眼内炎被确诊18例(2003年~2009年15例,2010年~2015年3例),总计发病率为0.3%.头孢呋辛预防眼内炎(RR=0.24,P<0.05;95%CI 0.07-0.83),归因危险度百分比为-3.18%(P<0.05;95%CI-3.43--3.93),归因危险分数为-0.76(P<0.05;95%CI-0.17--0.93)。头孢呋辛的成本为29.6元/0.75g(1支),平均眼内炎的成本为11356.26元/例。头孢呋辛的经济节省力为每286例节约2890.66元。结论前房内注射头孢呋辛降低了白内障术后眼内炎的发病率,并有较高的经济效益。Objective To evaluate the clinical and economical effect of intracameral injection of cefuroxime preventing postoperative endophthalmitis.Methods The incidence of endophthalmitis before using intracameral cefuroxime(2003~2009)and after(2010~2015)of endophthalmitis after cataract surgery was retrospectively studied.The protective effect of cefuroxime was evaluated with the relative risk(RR),the clinical impact of cefuroxime on endophthalmitis was evaluated with the attributable risk fraction(AF),the economical impact was evaluate with the number needed to treat(NNT)and the savings on the number.Results A total of 6004 cases of patients underwent cataract operation,including 3284 cases from 2003 to 2009,2720 cases from 2010 to 2015.A total of 18 cases of endophthalmitis was diagnosed and the microbial culture positive(15 cases from 2003 to 2009 and 3 cases from 2010 to 2015).The total incidence was 0.3%.Cefuroxime protected against endophthalmitis(RR=0.24,P<0.05,95%CI 0.07-0.83),its ARP was-3.18(P<0.05,95%CI-3.43--3.93),and its attributable risk fraction was-0.76(P<0.05;95%CI-0.17--0.93).The cost of cefuroxime was RMB 29.6/0.75 g(1 dose)and the average cost of endophthalmitis was RMB 11,356.26/per case.Meanwhile,NNT(number needed to treat)was 286 and its savings per 286 was 2890.66 yuan.Conclusion The cefuroxime reduced the incidence of endophthalmitis of cataract surgery and had a high clinical and economic impact on protecting cataract patients.
