摘　　要：目的探讨三维能量多普勒超声(3D-PDU)用于评估治疗婴幼儿血管瘤(IH)效果的价值。方法收集26例接受局部皮下注射博来霉素治疗的IH患儿,于治疗前和首次治疗3个月后行3D-PDU扫查,测量瘤组织体积(V)及瘤体内血流相关参数,包括血管指数(VI)、血流指数(FI)及血管血流指数(VFI);根据疗效将患儿分为有效组与无效组,比较治疗后2组之间及有效组治疗前、后上述参数的差异。结果首次治疗3个月后复查显示,有效组19例、无效组7例,总有效率为73.08%(19/26)。治疗3个月后,有效组V[3.54(2.05,4.25)ml]、VI(41.44±11.48)%及VFI(18.94±6.55)均低于治疗前[V 6.22(4.82,7.14)ml、VI(53.03±13.13)%及VFI 24.69±8.41,P均<0.05]和无效组[V 8.47(3.25,25.41)ml、VI(61.59±11.29)%及VFI 28.15±6.22,P均<0.05]。结论3D-PDU可准确测量治疗前后IH体积与血流参数变化,进而定量评估疗效。Objective To explore the value of three-dimensional power Doppler ultrasound(3D-PDU)for evaluation on therapeutic effect of infant hemangioma(IH).Methods Data of 26 children with IH treated with local injection of bleomycin were retrospectively analyzed.According to the therapeutic effect,the children were divided into effective group and ineffective group.3D-PDU was performed before and 3 months after the first treatment.The tumor tissue volume(V)and blood flow related parameters,including vascular index(VI),blood flow index(FI)and vascular blood flow index(VFI)were measured and compared between groups,as well as before and after treatment in effective group.Results After 3 months of local injection of bleomycin,there were 19 cases in effective group and 7 cases in ineffective group,and the total effective rate was 73.08%(19/26).Three months after the first treatment,V(3.54[2.05,4.25]ml),VI(41.44±11.48)%and VFI(18.94±6.55)in effective group were all lower than those in ineffective group V(8.47[3.25,25.41]ml),VI[61.59±11.29]%and VFI 28.15±6.22,all P<0.05),also lower than those in effective group before treatment(V 6.22[4.82,7.14]ml,VI[53.03±13.13]%,VFI 24.69±8.41,all P<0.05).Conclusion 3D-PDU could accurately assess the changes of tumor tissue volume and blood flow parameters before and after treatment,and quantitatively evaluate the therapeutic effect of IH.
