摘 要:目的探讨锥形束CT(CBCT)三维重建在颌骨后牙区种植术前骨质评估及术后效果评价中的应用价值。方法选取2018年7月至2019年12月在南通市口腔医院和南通市第二人民医院拟行颌骨后牙种植患者65例(73侧,86个位点),同时采用CBCT和曲面断层检查,比较2种影像学检查方法对牙槽骨骨量检出及颌骨颏孔区重要解剖结构显示情况,并对术后3个月骨结合情况进行评价。结果采用CBCT检查所发现的上颌后牙区牙槽骨骨量≥10 mm及下颌后牙区牙槽骨骨量≥12 mm的可植入位点数均明显多于曲面断层检查(P<0.05);CBCT对颏孔、切牙管、颏神经环以及颏棘上孔的检出效果均显著高于曲面断层(P<0.05);种植术后3个月,CBCT对种植术后未发生骨结合检出率为2.5%,显著低于曲面断层11.2%,差异有统计学意义(P<0.05)。结论在颌骨后牙区牙种植术前诊断、术后效果评估中应用CBCT三维重建技术,有助于提高可种植率,减少术后并发症的发生。Objective To investigate the application value of 3D digital reconstruction technology of CBCT on preoperative bone assessment and postoperative effect evaluation in mandibular posterior area implantation.Methods Sixty-five patients(73 sides,86 points)with posterior teeth implantation were selected in the Stomatological Hospital of Nantong and the Second People′s Hospital of Nantong between July 2018 and December 2019,all of them were examined by CBCT and panoramic radiograph.The detection of alveolar bone mass and the display of important anatomical structures in the mental foramen area were compared between the two imaging methods.The postoperative osseointegration was evaluated in 3 months after operation.Results The number of implantable sites with alveolar bone mass≥10 mm in maxillary posterior area and≥12 mm in mandibular posterior area detected by CBCT were significantly more than those detected by panoramic radiography(P<0.05).The detection effect of CBCT on mental foramen,incisor tube,mental nerve ring and supramental foramen were significantly higher than that of panoramic radiograph(P<0.05).At 3 months after implantation,the detection rate of CBCT for no osseointegration was 2.5%,which was lower than 11.2%of panoramic radiograph,and the difference was statistically significant(P<0.05).Conclusion The application of CBCT 3D digital reconstruction technology in the preoperative diagnosis and postoperative evaluation of dental implants in mandibular posterior area could help to improve the implant rate and reduce the incidence of postoperative complications.
