丁苯酞软胶囊对急性脑梗死患者血清miR-124表达和神经功能缺损程度评分的影响

摘　　要：目的:探讨丁苯酞软胶囊对急性脑梗死患者血清miR-124表达和神经功能缺损程度评分的影响。方法:选取2018年1月-2019年1月在监利市中医医院接受治疗的急性脑梗死患者118例,采用随机数字表法将118例患者随机分为常规治疗组、丁苯酞治疗组,每组59例。采用脑保护及抗血小板聚集或其他常规护理方法对常规治疗组进行治疗,丁苯酞治疗组在常规治疗组基础上联合丁苯酞软胶囊进行治疗;实时荧光定量检测法对miR-124表达进行检测;采用酶联免疫吸附实验法对血同型半胱氨酸(Hcy)、神经营养因子(NTF)、神经生长因子(NGF)、神经元特异性烯醇化酶(NSE)水平进行检测;使用改良的Rankin量表(mRS)评分、美国国立卫生研究院卒中量表(NIHSS)评分对患者神经功能损伤程度进行评价,对比两组在治疗过程中不良事件发生情况及治疗效果。结果:治疗后,丁苯酞治疗组FIB、D-D、Hcy、NSE水平、脑梗死面积及mRS、NIHSS评分均低于常规治疗组,NTF、NGF、miR-124水平均高于常规治疗组(P<0.05)。结论:通过丁苯酞软胶囊对急性脑梗死患者进行治疗,使患者的神经功能受损程度及炎症反应均得到有效降低,凝血功能得到改善,血液循环得到促进,显著降低脑血栓发生,减轻脑梗死面积。Objective:To investigate the effect of Butylphthalide Soft Capsules on serum miR-124 expression and the degree of neurological deficit score in patients with acute cerebral infarction.Method:A total of 118 patients with acute cerebral infarction treated in Jianli Hospital of Traditional Chinese Medicine from January 2018 to January 2019 were selected,and were divided into conventional treatment group and Butylphthalide treatment group by random number table method,with 59 patients in each group.The conventional treatment group was used brain protection and antiplatelet aggregation or other conventional caring method;the Butylphthalide treatment group was treated with Butylphthalide Soft Capsules on the basis of conventional treatment group.The miR-124 expression was measured by quantitative real-time fluorescence detection method;the levels of blood homocysteine(Hcy),neurotrophic factor(NTF)and nerve growth factor(NGF)and neuron-specific enolase(NSE)were detected by enzyme-linked immunosorbent assay;the degree of neurological impairment was evaluated by using modified Rankin scale(mRS)and National Institutes of Health stroke scale(NIHSS),the occurrence of adverse events during treatment and the treatment efficacy between the two groups were compared.Result:The levels of FIB,D-D,Hcy,NSE,cerebral infarction area and mRS and NIHSS scores in Butylphthalide treatment group were lower than those in conventional treatment group,while the levels of NTF,NGF and miR-124 were higher than those in conventional treatment group(P<0.05).Conclusion:When patients with acute cerebral infarction are treated with Butylphthalide Soft Capsules,the degree of nerve function damage and inflammatory response are effectively reduced,coagulation function is improved,blood circulation is promoted,significantly reducing the occurrence of cerebral thrombosis and reducing the area of cerebral infarction.
