摘　　要：目的:探讨p16/Ki-67检查在宫颈不典型鳞状上皮细胞(ASCUS)及低级别鳞状上皮内病变(LSIL)中的诊断价值。方法:回顾性选择2020年9月-2021年11月东莞市东部中心医院疑似ASCUS患者56例作为观察组。选择同期诊疗的疑似LSIL患者52例为对照组。所有患者均进行阴道镜检查活检及p16/Ki-67检查。以阴道镜活检为“金标准”,分析p16/Ki-67在ASCUS及LSIL中的诊断价值。结果:p16/Ki-67诊断ASCUS敏感度为84.44%,特异度为63.64%,阳性预测值为90.48%,阴性预测值为50.00%。p16/Ki-67诊断LSIL敏感度为82.93%,特异度为72.73%,阳性预测值为91.89%,阴性预测值为53.33%。p16/Ki-67诊断两种疾病敏感度、特异度、阳性预测值和阴性预测值比较差异均无统计学意义(P>0.05)。结论:p16/Ki-67用于ASCUS和LSIL患者中,能获得较高的筛查效能,诊断敏感度和特异度高,可为临床诊疗提供参考依据。Objective:To investigate the diagnostic value of p16/Ki-67 in cervical atypical squamous cell of undetermined signification(ASCUS)and low-grade squamous intraepithelial lesions(LSIL).Method:A total of 56 suspected ASCUS patients in Dongguan East Central Hospital from September 2020 to November 2021 were retrospectively selected as the observation group.52 suspected LSIL patients diagnosed and treated at the same time were selected as the control group.All patients underwent colposcopy biopsy and p16/Ki-67 examination.With colposcopy biopsy as the“gold standard”,the diagnostic value of p16/Ki-67 in ASCUS and LSIL was analyzed.Result:The sensitivity of p16/Ki-67 in the diagnosis of ASCUS was 84.44%,the specificity was 63.64%,the positive predictive value was 90.48%,and the negative predictive value was 50.00%.The sensitivity of p16/Ki-67 in the diagnosis of LSIL was 82.93%,the specificity was 72.73%,the positive predictive value was 91.89%,and the negative predictive value was 53.33%.There were no significant differences in the sensitivity,specificity,positive predictive value and negative predictive value of p16/Ki-67 in diagnosing the two diseases(P>0.05).Conclusion:p16/Ki-67 used in ASCUS and LSIL patients can obtain high screening efficiency,high diagnostic sensitivity and specificity,and can provide a reference for clinical diagnosis and treatment.
