摘　　要：目的:探究超声引导下微创旋切术治疗乳腺多发性肿块的效果及其对术后疼痛与应激反应的影响。方法:回顾性分析2018年1月-2021年12月新疆生产建设兵团第十三师红星医院收治的204例乳腺多发性肿块患者。根据手术方法的不同将其分为常规组(n=112)和微创组(n=92)。常规组接受开放型手术治疗,微创组采用超声引导下的微创旋切术。比较两组围手术期指标、切口愈合情况,手术前后应激指标,术后6、24、48、72 h及7 d视觉模拟疼痛(VAS)评分及并发症。结果:微创组术中出血量少于常规组,手术耗时、切口愈合时间、瘢痕长度均短于常规组(P<0.05)。微创组手术后血清肾上腺素(E)、皮质醇(Cor)水平均低于常规组(P<0.05)。术后6、24、48、72 h,微创组VAS评分均显著低于常规组(P<0.05),但两组术后7 d的VAS评分比较差异无统计学意义(P>0.05)。微创组术后并发症发生率为6.52%,显著低于常规组的21.43%(P<0.05)。结论:超声引导下微创旋切术治疗乳腺多发性肿块,手术耗时短,术中创伤小,可缩短切口愈合时间,减轻术后瘢痕,同时有助于减轻术后疼痛及应激反应,并降低并发症发生率。Objective:To explore the effect of ultrasound-guided minimally invasive circumcision in the treatment of multiple breast masses and its effect on postoperative pain and stress response.Method:A total of 204 patients with multiple breast masses admitted to Hongxing Hospital of the 13th Division of Xinjiang Production and Construction Corps from January 2018 to December 2021 were retrospectively analyzed.They were divided into routine group (n=112) and minimally invasive group (n=92) according to different surgical methods.The routine group received open surgery,and the minimally invasive group received ultrasound-guided minimally invasive circumcision.The perioperative indexes,wound healing condition,preoperative and postoperative stress indexes,visual analogue scale (VAS) scores at 6,24,48,72 hours and 7 days after surgery and complications were compared between the two groups.Result:The amount of intraoperative bleeding in the minimally invasive group was less than that in the routine group,and the surgery time,wound healing time and scar length were shorter than those in the routine group (P<0.05).The levels of serum adrenaline (E) and cortisol (Cor) in the minimally invasive group were lower than those in the routine group after surgery (P<0.05).At 6,24,48 and 72 hours after surgery,the VAS score of the minimally invasive group was significantly lower than that of the routine group (P<0.05),but there was no significant difference in VAS scores between the two groups at 7 days after surgery (P>0.05).The incidence of postoperative complications in the minimally invasive group was 6.52%,which was significantly lower than 21.43% in the routine group (P<0.05).Conclusion:Ultrasound-guided minimally invasive circumcision for the treatment of multiple breast masses has short surgery time and small intraoperative trauma.It can shorten the wound healing time,reduce postoperative scar,reduce postoperative pain and stress response,and reduce the incidence of complications.
