摘　　要：目的探讨多层螺旋CT鉴别恶性胸膜间皮瘤与胸膜转移瘤的应用价值分析。方法选取我院自2017年5月-2021年5月经病理学确诊的30例原发恶性胸膜间皮瘤及30例胸膜转移瘤患者的多层螺旋CT检查资料进行回顾性影像学分析。结果恶性胸膜间皮瘤患者与胸膜转移瘤患者CT表现环状胸膜增厚、叶间胸膜受累情况、胸膜斑、肺门及纵隔淋巴结肿大、胸廓体积缩小、心包受累、肺内转移情况,两者差异显著(P<0.05);胸腔积液、胸壁受累情况无显著差异(P>0.05)。结论环形胸膜增厚、叶间胸膜块状增厚、胸膜斑、患侧胸廓体积缩小、心包受累有利于恶性胸膜间皮瘤的诊断,而叶间胸膜多发小结节状转移,肺门及纵隔淋巴结肿大、肺内转移有利于胸膜转移瘤的诊断。Objective To explore the application value of multi-slice spiral CT in distinguishing malignant pleural mesothelioma and pleural metastasis.Methods The multi-slice spiral CT examination data of 30 patients with primary malignant pleural mesothelioma and 30 patients with pleural metastases who were pathologically diagnosed in our hospital from May 2017 to May 2021 were selected for retrospective imaging analysis.Results CT findings of patients with malignant pleural mesothelioma and pleural metastases included thickening of the annular pleura,involvement of the interlobular pleura,pleural plaques,enlarged hilar and mediastinal lymph nodes,reduced thoracic volume,pericardial involvement,and intrapulmonary metastasis.There was a significant difference between the two groups(P<0.05).There was no significant difference in pleural effusion and chest wall involvement(P>0.05).Conclusion Annular pleura thickening,massive thickening of interleaf pleura,pleural plaques,reduced thoracic volume on the affected side,and pericardial involvement are conducive to the diagnosis of malignant pleural mesothelioma,while multiple small nodular metastases in the interlobular pleura,hilar and mediastinal lymph nodes enlargement and intrapulmonary metastasis are conducive to the diagnosis of pleural metastases.
