原发性气管腺样囊性癌调强放射治疗的安全性和有效性分析

摘　　要：目的回顾性分析我院收治的原发性气管腺样囊性癌患者进行放射治疗的安全性和有效性。方法分析我院自2009年10月至2020年2月收治的22例原发性气管腺样囊性癌患者放射治疗的有效率,对放疗后复发和转移情况进行分析研究。结果22例患者中21例完成放疗计划。21例患者中镜下治疗联合放疗7例,其中1例CR,3例PR,总ORR为57.1%。单纯放疗10例,其中2例CR,2例PR,总ORR为40.0%。手术联合术后放疗组、镜下治疗联合放疗组及单纯放疗组的中位PFS分别为135.3月、40.3月和84.5月。截至随访日期,单纯放疗组和镜下治疗联合放疗组各1例患者死亡。结论原发性气管腺样囊性癌对放射治疗相对敏感,手术联合术后放疗、镜下治疗联合放疗或者单纯放疗均能获得较好的无进展生存期。Objective To investigate the safety and effectiveness of radiotherapy for patients with primary adenoid cystic carcinoma of trachea treated in our hospital.Methods From April 2009 to September 2016,22 patients with primary tracheal adenoid cystic carcinoma admitted to our hospital was analyzed,and the recurrence and metastasis after radiotherapy were analyzed and studied.Results Of the 22 patients,21 completed the radiotherapy schedule.In the 21 patients,7 cases were treated with endoscopic combined radiotherapy,including 1 case of CR and 3 cases of PR,and the total ORR was 57.1%.Radiotherapy alone was used in 10 cases,including 2 cases of CR and 2 cases of PR.The total ORR was 40.0%.The median PFS was 135.3,40.3,and 84.5 months in the surgery combined with postoperative radiotherapy group,the endoscopic treatment combined with radiotherapy group,and the radiotherapy alone group,respectively.End up to the follow-up date,one patient in the radiotherapy alone group and one patient in the combined radiotherapy group died.Conclusion Primary adenoid cystic carcinoma of trachea is relatively sensitive to radiotherapy,and surgery combined with postoperative radiotherapy or endoscopic treatment combined with radiotherapy and radiotherapy alone can achieve a better progression-free survival.
