摘　　要：目的探讨老年人血液中血管紧张素Ⅱ(angiotensinⅡ,AngⅡ)水平与认知功能及内侧颞叶萎缩(MTA)的相关性。方法共入组老年AD病人(AD组)40例,老年认知功能正常者(对照组)40例。所有参与者均完成中文版Addenbrooke认知评估量表Ⅲ(ACE-Ⅲ)、MMSE、日常生活能力评定量表(ADL)评估及24 h血压水平监测,并完成头颅MRI检查,予经典MTA评分。采用酶联免疫吸附法检测血浆AngⅡ水平。AngⅡ与认知功能及MTA评分的相关性采用Pearson相关系数描述。结果AD组的年龄、血浆AngⅡ水平、MTA评分显著高于对照组,MMSE、ACE-Ⅲ、ADL评分显著低于对照组,差异均有统计学意义(P<0.05),2组SBP、DBP水平差异无统计学意义(P>0.05)。AngⅡ水平与MMSE、ACE-Ⅲ、ADL评分无明显相关性(P>0.05),与MTA评分呈正相关性(r=0.228,P<0.05),且这种相关性与血压水平无关。结论老年AD病人血液中AngⅡ水平高于认知功能正常者,而且老年人AngⅡ水平升高与内侧颞叶萎缩具有一定相关性。Objective To investigate the associations of angiotensinⅡ(AngⅡ)level with cognitive function and medial temporal atrophy(MTA)in the elderly.Methods Forty elderly patients with Alzheimer’s disease(AD group)and 40 elderly with normal cognitive function(control group)were enrolled.All the subjects were evaluated by Chinese version of Addenbrooke’s Cognitive ExaminationⅢ(ACE-Ⅲ),Mini-Mental State Examination(MMSE)and Activities of Daily Living Scale(ADL),and 24-hour blood pressure was measured.All the subjects were rated by medial temporal atrophy(MTA)scale on MRI.The level of AngⅡwas measured by enzyme-linked immunosorbent assay.Pearson correlation analysis was performed to evaluate the associations of AngⅡlevel with cognitive function and MTA.Results The age,AngⅡlevel,MTA score were significantly higher,and the scores of MMSE,ACE-Ⅲand ADL were significantly lower in the AD group than those in the control group(P<0.05).There were no significant differences in the levels of systolic blood pressure and diastolic blood pressure between the two groups(P>0.05).The level of AngⅡwas not related to the scores of MMSE,ACE-Ⅲand ADL(P<0.05),but positively correlated with MTA(r=0.228,P<0.05),and further study showed the correlation was not mediated by blood pressure.Conclusions The level of AngⅡin the elderly patients with AD is higher than that in the normal elderly,and the increased level of AngⅡis associated with MTA.
