血清胰岛素样生长因子-1、红细胞分布宽度及糖类抗原199在食管癌诊断及病情评估中的作用

摘　　要：目的分析胰岛素样生长因子-1(IGF-1)、红细胞分布宽度(RDW)、糖类抗原199(CA199)在食管癌诊断及病情评估中的作用。方法2017年1月至2020年6月我院收治的食管癌患者61例(观察组),TNM分期I~II期35例,III~IV期26例;选取同期58例健康体检者作为健康组。对比不同人群、不同分期及预后患者中IGF-1、RDW、CA199表达情况,分析IGF-1、RDW、CA199对食管癌的诊断价值。结果观察组IGF-1、RDW、CA199表达高于健康组(P<0.05);III~IV期患者IGF-1、RDW、CA199表达高于I-II期患者(P<0.05);随访1年,观察组预后良好47例,预后不良14例,预后不良患者IGF-1、RDW、CA199表达高于预后良好者(P<0.05)。IGF-1+RDW+CA199联合检测对食管癌早期诊断敏感度和特异度分别为88.20%、79.20%,AUC=0.838(95%CI0.723~0.896),高于IGF-1、RDW、CA199单独预测(P<0.05)。结论IGF-1、RDW、CA199与食管癌病情及预后情况密切相关,可为患者手术治疗选择及预后评估提供参考信息。Objective To analyze the role of insulin like growth factor-1(IGF-1),erythrocyte distribution width(RDW)and carbohydrate antigen 19-9(CA199)in the diagnosis and disease assessment of esophageal cancer.Methods Sixty-one patients with esophageal cancer admitted to our hospital from January 2017 to June 2020 were collected as an observation group,in which there were 35 with TNM stage I~II and 26 with TNM III~IV.During the same period,58 healthy subjects were selected as a healthy group.The expressions of IGF-1,RDW and CA199 in patients with different population,stages and prognosis were compared,and the diagnostic value of IGF-1,RDW and CA199 for esophageal cancer was analyzed.Results The expression levels of IGF-1,RDW and CA199 in the observation group were higher than those in the healthy group(P<0.05).The expression levels of IGF-1,RDW and CA199 in stage III~IV were significantly higher than those in stage I~II(P<0.05).The patients were followed up for 1 year.Among the 61 patients,47 had good prognosis and 14 had poor prognosis.The expressions of IGF-1,RDW and CA199 in parents with poor prognosis were significantly higher than those in patients with good prognosis(P<0.05).According to ROC curve analysis,the sensitivity and specificity of combined detection of IGF-1+RDW+CA199 for early diagnosis of esophageal cancer were 88.20%and 79.20%,respectively,AUC=0.838(95%CI:0.723~0.896),which were significantly higher than that of IGF-1,RDW and CA199 detection alone(P<0.05).Conclusion IGF-1,RDW and CA199 are closely related to the condition and prognosis of esophageal cancer.The indexes can provide reference information for surgical treatment selection and prognosis evaluation of the patients.
