摘 要:目的分析丝裂霉素C(MMC)在兔眼小梁切除中房水、玻璃体及静脉血液中残存药物浓度分布规律;比较分别放置于结膜瓣、巩膜瓣时前房水、玻璃体及静脉血液中MMC药物浓度的差异。方法新西兰大白兔60只,分为两组,均行小梁切除术,术中应用同等剂量MMC。结膜组将MMC置于结膜瓣下,巩膜组置于巩膜瓣下。各组分别于5、30、60min选择10只兔子,抽取房水及玻璃体各0.12mL、静脉血3mL迅速行高效液相色谱分析,测量样本中MMC药物浓度。结果术中应用MMC后,结膜组房水、玻璃体、静脉血中MMC浓度峰值分别为(27.26±6.33)ng/mL、(2.30±0.76)ng/mL、(35.32±17.26)ng/mL,巩膜组分别为(74.52±6.27)ng/mL、(3.57±0.93)ng/mL、(43.31±12.96)ng/mL,两组比较差异均有统计学意义(P<0.05)。两组药物峰值持续时间:房水为30min,玻璃体与静脉血中为5min。结论兔眼小梁切除术中应用低浓度MMC,房水、玻璃体及静脉血中高峰时期药物浓度均低于造成组织损害的临界值;房水、玻璃体、静脉血液中MMC药物浓度峰值出现时间分别为30、5、5min;巩膜瓣放置MMC较结膜瓣放置会造成更高的药物浓度残留,需要更长的时间代谢药物。Objective To analyze the distribution of residual mitomycin C concentration in aqueous humor,vitreous body,and venous blood during trabeculectomy in rabbits;to compare the concentration difference of MMC in anterior aqueous humor,vitreous and venous blood samples when MMC were placed at conjunctival flap and scleral flap.Methods Sixty New Zealand white rabbits were divided into two groups.All rabbits underwent trabeculectomy.The same dose of mitomycin C was used during the surgery.Mitomycin C was placed under the conjunctival flaps or scleral flaps in conjunctival group or scleral group,respectively;0.12mL aqueous humor,0.12mL vitreous body and 3mL venous blood were extracted in ten rabbits at 5 minutes,30 minutes and 60 minutes after treatment in two groups,respectively.Mitomycin C concentration were detected by High-Performance Liquid Chromatography in these samples.Results The highest mitomycin C concentrations in aqueous humor,vitreous and venous blood were(27.26±6.33)ng/mL,(2.30±0.76)ng/mL,(35.32±17.26)ng/mL(conjunctival group),(74.52±6.27)ng/mL,(3.57±0.93)ng/mL and(43.31±12.96)ng/mL(scleral group),the concentrations difference between the two groups was statistically significant(P<0.05);The peak time of drug concentration in aqueous humor,vitreous and venous blood was 30 minutes,5 minutes and 5 minutes,respectively.Conclusion The peak mitomycin C concentrations in aqueous humor,vitreous,and venous blood in rabbits trabeculectomy,were detected at 30 minutes,5 minutes,and 5 minutes after treatment,respectively;with low dose mitomycin C in rabbits trabecu-lectomy,the highest concentration in aqueous humor,vitreous and venous blood was lower than the critical value of tissue damage;Compared with conjunctival flap,the application on scleral flap will cause higher residual concentration,and take longer to drug metabolize Mitomycin C.
