中性粒细胞、淋巴细胞、中性粒细胞与淋巴细胞比值在急性脑梗死中的临床分析

摘　　要：目的分析外周血中性粒细胞计数、淋巴细胞计数、中性粒细胞与淋巴细胞比值(NLR)对急性脑梗死(ACI)中的诊断价值及其与ACI患者病情严重程度的相关性。方法选取苏州大学附属第一医院收治的ACI患者60例(ACI组)和健康体检者30名(对照组),检测发病24 h内外周血中性粒细胞和淋巴细胞的数量,计算NLR。同时统计ACI患者入院后24 h内的NIHSS评分。比较两组中性粒细胞计数、淋巴细胞计数、NLR的差异,将差异有统计学意义的指标进行ROC曲线分析,评估其对ACI的诊断价值。进一步采用Pearson直线相关分析差异有统计学意义的指标与NIHSS评分的相关性。结果血清中性粒细胞计数与NLR在ACI患者中水平上升,与对照组间差异有统计学意义(P<0.05或P<0.01)。而淋巴细胞计数在两组间差异无统计学意义(P>0.05)。ROC曲线分析显示中性粒细胞计数及NLR对ACI诊断均具有预测价值,中性粒细胞计数与NIHSS评分成正相关(r=0.705,P<0.01);淋巴细胞计数与NIHSS评分无相关性(P>0.05)。结论早期外周血中性粒细胞计数、NLR可以用于ACI的辅助诊断,中性粒细胞计数亦可以用于ACI疾病严重程度的辅助评估。Objective To analyze the diagnostic value of neutrophil count,lymphocyte count,neutrophil to lymphocyte ratio(NLR)in peripheral blood for acute cerebral infarction(ACI)and its correlation with the severity of ACI patients.Methods 60 patients with ACI(ACI group)and 30 healthy subjects(control group)were selected to detect the number of peripheral blood neutrophils and lymphocytes and NLR was calculated within 24 hours of onset.NIHSS scores of ACI patients within 24 hours after admission were counted.The differences between the two groups were compared.ROC curve was used to analyze the diagnostic value of blood neutrophils,lymphocytes and NLR in ACI.Pearson linear correlation was used to analyze the correlation among neutrophils number,lymphocyte number,NLR and NIHSS score.Results Serum neutrophils and NLR increased in ACI patients.There were statistically significant differences between ACI group and control group(P<0.05 or P<0.01).Lymphocyte showed no significant difference between the two groups(P>0.05).ROC curve analysis showed that both neutrophils and NLR had predictive value for ACI diagnosis.There was a good correlation between neutrophils and NIHSS score(r=0.705,P<0.01).There was no correlation between lymphocyte count and NIHSS score(P>0.05).Conclusion Early neutrophils and NLR can be used in the auxiliary diagnosis of ACI,and neutrophils count can also be used in the auxiliary assessment of the severity of ACI.
