老年帕金森病患者体位性低血压发生的危险因素分析

摘　　要：目的探寻可能影响老年帕金森病(Parkinson’s disease,PD)患者发生体位性低血压的危险因素。方法采集四川大学华西医院神经内科2019年4月—2021年3月期间收治的70例老年PD伴体位性低血压患者的临床资料,另采集同时期内医院收治的70例未合并体位性低血压的老年PD患者的临床资料纳入对照组,探寻可能影响老年PD患者发生体位性低血压的因素。结果与对照组相比,合并体位性低血压组患者的年龄较大,合并糖尿病、体质量指数(BMI)≤18.4kg/m^(2)、25-羟基维生素D(25-OH-VD)<20μg/L和蒙特利尔认知评定量表(MOCA)评分<26分的构成比较高,自主神经症状量表(SCOPA-AUT)评分较高(P<0.05)。经Logistic回归模型多因素分析结果显示,年龄、合并糖尿病、BMI、25-OH-VD、SCOPA-AUT评分、MOCA评分均是老年PD患者发生体位性低血压的影响因素(OR>1,P<0.05)。结论老年PD患者发生体位性低血压受多个危险因素影响,应开展相应干预措施,以期降低PD患者体位性低血压的发生风险。Objective To explore the risk factors that may affect the occurrence of postural hypotension in elderly patients with Parkinson’s disease(PD).Methods The clinical data of 70 elderly PD patients with postural hypotension treated in the Department of Neurology of West China Hospital from April 2019 to March 2021 were collected.In addition,the clinical data of 70 elderly PD patients without postural hypotension treated in the hospital during the same period were collected as the control group.The factors that might affect the occurrence of postural hypotension in elderly PD patients were analyzed.Results Compared with the control group,the age of the postural hypotension group was older and the constituent ratio of combined diabetes,body mass index(BMI)≤18.4kg/m^(2),25-hydroxyvitamin D(25-OH-VD)<20μg/L and the Montreal Cognitive Assessment Scale(MoCA)score<26 points of the postural hypotension groupwere higher,and the Autonomic Symptom Scale(SCOPA-AUT)score of the postural hypotension groupwas higher(P<0.05).Logistic regression model multivariate analysis showed that age,combined diabetes,BMI,25-OH-VD,SCOPA-AUT score and MOCA score were all influencing factors of postural hypotension in elderly PD patients(OR>1,P<0.05).Conclusion The occurrence of postural hypotension in elderly PD patients is affected by many risk factors,and correspondingintervention measures should be carried out to reduce the risk of postural hypotension in elderly PD patients.
