经腋窝入路在甲状腺微小癌腔镜手术中的应用效果及对生存质量的影响研究

摘　　要：目的:观察及比较经胸乳入路与经腋窝入路在甲状腺微小癌腔镜手术中的应用效果及对生存质量的影响。方法:选取2019年6月-2021年3月龙岩市第二医院收治的80例拟行甲状腺微小癌腔镜手术患者,根据随机数字表法将其分为A组(经胸乳入路)和B组(经腋窝入路),每组40例。比较两组手术时间、术中出血量、术后引流量、美观满意度、视觉模拟评分法(VAS)及生存质量评分(SF-36量表)。结果:B组手术时间短于A组,术中出血量及术后引流量均少于A组,美观程度优于A组,差异均有统计学意义(P<0.05)。术后3、7、14 d,B组VAS评分均优于A组,差异均有统计学意义(P<0.05)。术前,两组SF-36量表各维度评分比较,差异均无统计学意义(P>0.05);术后1、4周,B组SF-36量表各维度评分均优于A组,差异均有统计学意义(P<0.05)。结论:经腋窝入路在甲状腺微小癌腔镜手术中的应用效果相对优于经胸乳入路,且对生存质量的影响更好,因此在甲状腺微小癌患者中的应用价值相对较高。Objective:To observe and compare the application effect of transthoracic approach and transaxillary approach in endoscopic surgery for thyroid microcarcinoma and its influence on quality of life.Method:A total of 80 patients with thyroid microcarcinoma underwent endoscopic surgery who were admitted to the Second Hospital of Longyan from June 2019 to March 2021 were selected,and they were divided into group A(transthoracic approach)and group B(transaxillary approach)according to random number table method,with 40 patients in each group.Surgical time,intraoperative blood loss,postoperative drainage volume,aesthetic satisfaction,visual analogue scale(VAS)score and quality of life score(SF-36 scale)were compared between two groups.Result:The surgical time of the group B was shorter than that of the group A,the amount of intraoperative blood loss and postoperative drainage of the group B were less than those of the group A,and the aesthetic degree of the group B was better than that of the group A,the differences were statistically significant(P<0.05).3,7 and 14 d after surgery,the VAS scores of the group B were better than those of the group A,the differences were statistically significant(P<0.05).Before surgery,there were no significant differences in SF-36 scale scores of all dimensions between two groups(P>0.05);1 week and 4 weeks after surgery,SF-36 scale scores of all dimensions of the group B were better than those of the group A,the differences were statistically significant(P<0.05).Conclusion:The application effect of transaxillary approach in endoscopic surgery for thyroid microcarcinoma is relatively better than that of transthoracic approach,and has a better impact on the quality of life,therefore,the application value in patients with thyroid microcarcinoma is relatively high.
