早期应用低分子肝素对重症脑出血患者的效果评价

摘　　要：目的分析早期应用低分子肝素(LMWH)对于重症脑出血患者的效果。方法100例重症脑出血患者,以随机法分为A组(52例)和B组(48例)。B组采取常规治疗,A组采取LMWH治疗。对比两组患者的临床疗效、下肢周径与静脉血液流速、下肢静脉血栓发生情况、不良反应发生情况。结果A组的总有效率为98.08%,显著高于B组的85.42%,差异具有统计学意义(P<0.05)。治疗前,两组的下肢周径、股静脉血液流速、腘静脉血液流速对比差异均无统计学意义(P>0.05);治疗后,A组的下肢周径(30.12±2.43)cm小于B组的(31.99±2.41)cm,股静脉血液流速(28.26±3.98)cm/s、腘静脉血液流速(21.39±3.55)cm/s均大于B组的(21.47±3.85)、(16.87±3.42)cm/s,差异具有统计学意义(P<0.05)。A组的下肢静脉血栓发生率为3.85%,低于B组的18.75%,差异具有统计学意义(P<0.05)。A组的不良反应发生率为5.77%,低于B组的20.83%,差异具有统计学意义(P<0.05)。结论重症脑出血患者早期应用LMWH治疗能够改善临床症状,缩小下肢周径,恢复股静脉与腘静脉血流,能够预防下肢静脉血栓与不良反应。Objective To analyze the effect of early application of low molecular weight heparin(LMWH)on patients with severe cerebral hemorrhage.Methods A total of 100 patients with severe cerebral hemorrhage were randomly divided into group A(52 cases)and group B(48 cases).Group B was treated with conventional therapy,and group A was treated with LMWH.Both groups were compared in terms of clinical efficacy,lower extremity circumference and venous blood flow rate,the occurrence of lower extremity venous thrombosis,and the occurrence of adverse reactions.Results The total effective rate of group A was 98.08%,which was significantly higher than 85.42%of group B,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in lower extremity circumference,femoral vein blood flow velocity,and popliteal vein blood flow velocity between the two groups(P>0.05).After treatment,the lower extremity circumference(30.12±2.43)cm of group A was less than(31.99±2.41)cm of group B;the femoral vein blood flow velocity(28.26±3.98)cm/s,and the popliteal vein blood flow velocity(21.39±3.55)cm/s were greater than(21.47±3.85)and(16.87±3.42)cm/s in group B;all the differences were statistically significant(P<0.05).The incidence of lower extremity venous thrombosis in group A was 3.85%,which was lower than 18.75%in group B,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in group A was 5.77%,which was lower than 20.83%in group B,and the difference was statistically significant(P<0.05).Conclusion Early application of LMWH in patients with severe cerebral hemorrhage can improve clinical symptoms,reduce the circumference of the lower extremity,restore the blood flow of the femoral vein and popliteal vein,and prevent venous thrombosis and adverse reactions in the lower extremity.
