翼状胬肉切除联合自体球结膜瓣移植术中的临床应用

摘　　要：目的 探讨翼状胬肉切除联合自体球结膜瓣移植术治疗翼状胬肉疾病的临床治疗效果。方法本次研究中所纳入的研究对象为临泽县人民医院收治的翼状胬肉患者,一共40例,选取的时间段为2017年9月-2019年9月,按照不同的治疗方式将其分为参照组与研究组,每组20例,参照组患者应用翼状胬肉切除术,研究组患者采用翼状胬肉切除联合自体球结膜瓣移植术。观察两组患者术后疼痛程度、临床治疗疗效等相关情况。结果 参照组患者术后第1天、第3天VAS评分,明显高于研究组患者(P <0.05);术后随访6~18个月,研究组患者治愈率为90.00%,明显高于参照组70.00%(P <0.05)。两组各项生活质量评分差异较小(P>0.05),与干预后对照组比较,观察组角色受限、躯体疼痛、生命活力、感情因素、社交能力、精神健康、总体健康和活动能力评分均显著偏高(P <0.05)。结论 应用翼状胬肉切除联合自体球结膜瓣移植术治疗翼状胬肉患者,临床疗效非常显著,能有效缓解患者的临床症状,且该方式具有简便操作、方便取材、术后患者的反应比较轻、恢复速度快等优势,同时也是减少低翼状胬肉术后发生复发率的治疗方式之一。Objective To explore the clinical effect of pterygium resection combined with autologous bulbar conjunctival flap transplantation in the treatment of patients with pterygium disease.Methods The subjects included in this study were pterygium patients admitted to our hospital, with a total of 40 cases. The selected time period was from September 2017 to September 2019, and they were divided into reference group and study group according to different treatment methods. There were 20cases in each group, the control group was treated with pterygium excision, and the study group was treated with pterygium excision combined with autologous bulbar conjunctival flap transplantation.The postoperative pain degree and clinical treatment efficacy of the two groups were observed. Results The VAS scores of the patients in the reference group on the 1st and 3rd days after operation were significantly higher than those in the study group(P <0.05). After follow-up for 6~18 months, the cure rate of the study group was 90.00%, which was significantly higher than that of the reference group(70.00%)(P < 0.05). There was little difference in the scores of quality of life between the two groups(P > 0.05). Compared with the control group after intervention, the scores of role limitation, physical pain, vitality, emotional factors, social ability, mental health, general health and activity ability in the observation group were significantly higher(P <0.05). Conclusion Pterygium excision combined with autologous bulbar conjunctival flap transplantation in the treatment of pterygium patients has a very significant clinical effect, which can effectively alleviate the clinical symptoms of patients. This method has the advantages of simple operation, convenient material selection, light postoperative response and fast recovery. At the same time, it is also one of the treatment methods to reduce the recurrence rate of low pterygium.
