超声引导膈神经阻滞合水针治疗放化疗后顽固性呃逆的疗效观察

摘　　要：目的 探讨上消化道恶性肿瘤同步放化疗后所致的顽固性呃逆患者予以超声引导膈神经阻滞联合水针治疗的疗效及安全性。方法 将甘肃省武威市凉州医院2017年1月-2020年10月收治的上消化道恶性肿瘤因同步放化疗治疗后所致顽固性呃逆患者47例,随机分组,对照组予以山莨菪碱、氯丙嗪、中医疗法等单药或联合治疗,研究组予以超声引导膈神经阻滞联合水针治疗,观察两组治疗效果。结果 研究组总有效率96.0%显著高于对照组的56.5%,其不良反应率37.5%、复发率13.0%显著低于对照组的69.5%和30.8%,均具有统计学差异(P<0.05)。结论 上消化道恶性肿瘤同步放化疗所致的顽固性呃逆,成因复杂,治疗棘手,而超声引导膈神经阻滞联合水针治疗,具有安全高效、低复发、不良反应少等特点,值得临床借鉴。Objective To investigate the efficacy and safety of ultrasound-guided phrenic nerve block combined with water acupuncture in the treatment of intractable hiccup caused by concurrent radiotherapy and chemotherapy of upper gastrointestinal malignant tumors.Methods Forty-seven patients with intractable hiccup caused by concurrent chemoradiotherapy for upper gastrointestinal malignant tumors treated in our hospital from January 2017 to October 2020 were randomLy divided into two groups.The control group was treated with anisodamine,chlorpromazine,traditional Chinese medicine and other single or combined drugs.The study group was treated with ultrasound-guided phrenic nerve block combined with water acupuncture,and the therapeutic effects of the two groups were observed.Results The total effective rate of 96% in the study group was significantly higher than 56.5% in the control group,and the adverse reaction rate of 37.5% and recurrence rate of 13.0% were significantly lower than 69.5% and 30.8% in the control group (P<0.05).Conclusion Intractable hiccup caused by concurrent radiotherapy and chemotherapy of upper gastrointestinal malignant tumors has complex causes and difficult treatment.Ultrasound-guided phrenic nerve block combined with water acupuncture has the characteristics of safety,high efficiency,low recurrence and less adverse reactions,which is worthy of clinical reference.
