脑脊液和血清中PCT、CK-BB、LDH在小儿颅内感染期间的含量变化及临床价值

摘　　要：目的探讨颅内感染患儿脑脊液及血清PCT、CK-BB、LDH含量变化,以分析其在颅内感染中的诊断及鉴别意义。方法收集我院2018年3月-2021年2月收治的100例颅内感染患儿为研究对象,其中化脓性脑膜炎45例,病毒性脑炎55例,另选同期住院的45例热性惊厥儿童作为对照组。采集三组患儿急性期及恢复期脑脊液与静脉血,检测血清及脑脊液中PCT、CK-BB、LDH水平。结果急性期化脓性脑膜炎脑脊液及血清中PCT含量显著高于病毒性脑膜炎及对照组,差异有统计学意义(P<0.05);病毒性脑膜炎脑脊液及血清中PCT含量与对照组比较,差异无统计学意义(P>0.05);化脓性脑膜炎组和病毒性脑膜炎组血清及脑脊液中CK-BB、LDH水平显著高于对照组(P<0.05);脑脊液中,化脓性脑膜炎组LDH水平较病毒性脑膜炎高(P<0.05),血清中,化脓性和病毒性脑膜炎LDH水平比较差异无统计学意义(P>0.05);恢复期化脓性脑膜炎组患儿脑脊液及血清中PCT、CK-BB、LDH含量均较病毒性脑膜炎组高,差异有统计学意义(P<0.05)。结论检测脑脊液与血清PCT、LHD、CK-BB水平有利于早期鉴别化脓性脑膜炎和病毒性脑膜炎,动态监测上述指标对颅内感染患儿病情监测和预后评估有重要价值。Objective To investigate the changes in cerebrospinal fluid and serum PCT,CK-BB,and LDH levels in children with intracranial infection,in order to analyze its diagnostic and differential significance in intracranial infection.Methods A total of 100 children with intracranial infection admitted to our hospital from March 2018 to February 2021 were collected as the research objects,including 45 cases of purulent meningitis,55 cases of viral encephalitis,and 45 cases of fever who were hospitalized during the same period children with convulsions served as a control group.The cerebrospinal fluid and venous blood of the three groups of children in the acute and convalescent phases were collected to detect the levels of PCT,CK-BB,and LDH in the serum and cerebrospinal fluid.Results Acute phase:PCT content in cerebrospinal fluid and serum of purulent meningitis was significantly higher than that in viral meningitis and the control group,and the difference was statistically significant(P<0.05).Comparison of PCT content in cerebrospinal fluid and serum of viral meningitis and control group,the difference was not statistically significant(P>0.05).The levels of CK-BB and LDH in serum and cerebrospinal fluid in the purulent meningitis group and viral meningitis group were significantly higher than those in the control group(P<0.05).In cerebrospinal fluid,the level of LDH in purulent meningitis group was higher than that in viral meningitis(P<0.05).There was no significant difference in the level of LDH between purulent meningitis and viral meningitis in the serum(P>0.05).Recovery period:The contents of PCT,CK-BB and LDH in cerebrospinal fluid and serum of children in purulent meningitis group were higher than those in viral meningitis group.Conclusion The detection of cerebrospinal fluid and serum PCT,LHD,CK-BB levels is helpful for the early identification of purulent meningitis and viral meningitis.Dynamic monitoring of the above indicators is of great value for the monitoring and prognostic evaluation of children with int
