丁苯酞联合大活络胶囊治疗脑梗塞偏瘫疗效分析

摘　　要：目的研究脑梗塞偏瘫使用丁苯酞联合大活络胶囊治疗的临床效果。方法自2020年5月-2021年5月在本院选取60例脑梗塞偏瘫患者,使用随机数字表法将患者分为两组,各30例,采取丁苯酞治疗为对照组,采取丁苯酞联合大活络胶囊为实验组,对比两组治疗后临床效果,对比两组血液流变学指标[血小板聚集指数、血浆纤维蛋白原(FIB)、红细胞聚集指数],使用美国国立卫生院神经功能缺损评分(NIHSS)评估两组神经功能缺损情况。使用Fuel-Meyer评价法(FMA)评估患者运动功能。结果对比对照组,实验组总有效率较高(P<0.05);治疗前两组血小板聚集指数、FIB、红细胞聚集指数对比无差异(P>0.05),治疗后与对照组相比,实验组指标均低(P<0.05);治疗前两组NIHSS、FMA上肢运动功能与下肢运动功能评分对比无差异(P>0.05),治疗后,相较于对照组,实验组NIHSS评分低,FMA各项评分均高(P<0.05)。结论对脑梗塞偏瘫患者给予丁苯酞联合大活络胶囊治疗可提升疗效,改善其血液流变学指标,减轻神经功能缺损程度,进而改善预后,促进其患肢运动功能恢复,值得借鉴。Objective To study the clinical effectiveness of butyphthalide plus Dahuoluo capsules in treating the cerebral infarction with hemiplegia.Methods 60 cerebral infarction patients with hemiplegia treated from May 2020 to May 2021 in our hospital were selected.According to random number table,30 cases of taking butyphthalide were set as control group;30 cases of taking butyphthalide and Dahuoluo capsules were set as experimental group.Clinical effectiveness and hemorheological indicators like platelet aggregation,plasma fibrinogen(FIB)and erythrocyte aggregation indexes were compared;patient’s neurologic impairment conditions were appraised through national institute of health stroke scale(NIHSS),motor function by Fuel-Meyer Assessment(FMA).Results Total effective rate in the experimental group was higher than control group(P<0.05);before treatment,the platelet aggregation,FIB and erythrocyte aggregation indexes between groups were not significantly different(P>0.05);after treatment,related indicators in the experimental group were lower than control group(P<0.05);before treatment,NIHSS scores and FMA-upper extremity scale and FMA-lower extremity scale scores between groups were not significantly different(P>0.05);after treatment,NIHSS scores in the experimental group were lower than control group,but FMA scores were higher than control group(P<0.05).Conclusion Butyphthalide combined with Dahuoluo capsules can reduce the neurologic impairment,promote the motor function recovery,and improve the hemorheological indicators,treatment effectiveness and prognosis.
