摘　　要：目的探讨皮内连续缝合技术在低位直肠癌预防性回肠造口术中的应用效果。方法回顾性分析108例行低位直肠癌低位前切除术及预防性回肠造口术的患者的临床资料。根据造口缝合方式将患者分为观察组和对照组,各54例。观察组患者采用皮内连续缝合回肠造口,对照组患者采用传统回肠黏膜与皮肤间断缝合回肠造口。比较两组患者的造口手术时间、术后心理状态及造口术相关并发症发生情况。结果观察组的造口手术时间短于对照组(P<0.05)。术后6个月,两组患者焦虑、悲观、自卑、恐惧、社交障碍的发生率比较,差异均无统计学意义(均P>0.05)。术后12个月,观察组患者造口术相关并发症的发生率低于对照组(P<0.05)。结论与传统间断缝合技术相比,采用皮内连续缝合技术进行回肠造口可缩短造口手术时间,降低造口术相关并发症的总发生率,患者更容易接受。Objective To explore the application effect of continuous intradermal suture technique in prophylactic ileostomy for low rectal cancer.Methods The clinical data of 108 patients with low rectal cancer undergoing low anterior resection and prophylactic ileostomy were retrospectively analyzed.The patients were assigned to observation group or control group according to the suture methods of ostomy,with 54 cases in each group.Patients in the observation group employed ileostomy with continuous intradermal suture,whereas patients in the control group used ileostomy with conventional interrupted suture of ileum mucosa and skin.Operation duration of ostomy,postoperative psychological state,and the incidence of ileostomy-related complications were compared between the two groups.Results The observation group exhibited a shorter operation duration of ostomy as compared with the control group(P<0.05).Six months after operation,there were no statistically significant differences in the incidence rate of anxiety,pessimism,inferiority complex,phobia,or social communication disorder between the two groups(all P>0.05).Twelve months after operation,the observation group obtained a lower incidence rate of ileostomy-related complications in comparison with the control group(P<0.05).Conclusion Compared with conventional interrupted suture technique,applying continuous intradermal suture technique to conducting ileostomy can shorten operation duration of ostomy and decrease the overall incidence rate of ileostomy-related complications,which can be accepted by the patients more easily.
