摘　　要：目的:观察个体化经卵圆孔穿刺半月节射频热凝治疗三叉神经痛的效果。方法:对168例三叉神经痛患者行半月节射频热凝治疗,采用Hartel入路穿刺,用自制双栅栏定位器联合CT重建行个性化定位。结果:所有患者均经卵圆孔穿刺半月节成功。对热凝术后治愈的162例患者进行随访,治愈率按照生存率分析方法计算。1年、2年、3年、4年治愈率分别为93.79%,86.18%,83.55%和83.55%。并发症方面,角膜麻痹发生率3.70%,面部麻木发生率为100.00%,咀嚼无力发生率2.47%。结论:自制双栅栏定位器置于患侧面部联合CT重建可实现个性化确定穿刺点及穿刺方向,穿刺成功率达100%,疗效确切,可避免出现严重并发症。Objective:The effect of radiofrequency thermocoagulation on trigeminal neuralgia was analyzed retrospectively.Methods:A total of 162 patients with successful radiofrequency therapy through foramen ovale were followed up,including cure rate,complication rate,etc.The long term clinical effects were assessed with life-span table analysis.Results:The 1,2,3,and 4-year cure rates were respectively 93.79%,86.18%,83.55%and 83.55%.Among the complications,the incidence of corneal paralysis was 3.70%,facial numbness was 100%,masticatory weakness was 2.47%.Conclusion:The self-made fence locators combined with CT guidance can realize individual localization of puncture point and puncture direction,with high cure rate and no serious complications.
