胆囊占位性病变彩色多普勒超声图像及诊断价值分析

摘　　要：目的分析胆囊占位性病变的彩色血流征象,并探讨彩色多普勒超声对胆囊占位性病变的诊断价值。方法收集本院2018年11月—2021年12月收治的64例胆囊占位性病变临床资料。观察胆囊占位性病变血流分级、收缩期最大流速(V_(max))、阻力指数(RI)等情况;以病理结果为金标准,分析彩色多普勒超声诊断胆囊占位性病变的价值。结果64例胆囊占位性病变中,良性病变26例(良性组),恶性病变38例(恶性组)。良性组肿瘤直径短于恶性组,V_(max)、RI值、穿支血流检出率及≥2级血流信号显示率明显低于恶性组(P<0.05,P<0.01)。彩色多普勒超声诊断胆囊占位性病变的敏感度、特异度及准确度分别为81.58%、96.15%、87.50%;与病理结果具有良好的一致性,Kappa值=0.75。结论彩色多普勒超声可有效显示胆囊占位性病变的影像学特征,具有较高的诊断准确率,可为临床鉴别诊断提供可靠依据。Objective To analyze the color flow signs of gallbladder space-occupying lesions,and to explore the value of color Doppler ultrasonography in the diagnosis of gallbladder space-occupying lesions.Methods The clinical data of 64 patients with gallbladder space-occupying lesions admitted to our hospital from November 2018 to December 2021 were collected.The blood flow grading,systolic maximum velocity(V_(max))and resistance index(RI)of gallbladder space-occupying lesions were observed.With pathological results as the gold standard,the value of color Doppler ultrasonography in diagnosing gallbladder space-occupying lesions was analyzed.Results Among the 64 cases of gallbladder space-occupying lesions,there were 26 benign lesions(benign group)and 38 malignant lesions(malignant group).The tumor diameter of the benign group was shorter than that of the malignant group,and the V_(max),RI value,the detection rate of perforator blood flow and the display rate of blood flow signal≥grade 2 were significantly lower than those of the malignant group(P<0.05,P<0.01).The sensitivity,specificity and accuracy of color Doppler ultrasonography in diagnosing gallbladder space-occupying lesions were 96.15%,81.58%,and 87.50%,respectively,which were in line with pathological results.The Kappa value was 0.75.Conclusion Color Doppler ultrasonography can effectively display the imaging features of gallbladder space-occupying lesions,and has a high diagnostic accuracy,which,therefore,can provide a reliable basis for clinical differential diagnosis.
