抗抑郁药在消化内科应用的处方分析

摘　　要：目的分析消化内科门诊抗抑郁药的使用情况及用药的合理性,旨在为消化内科提供抗抑郁药的用药注意事项及药学监护的参考资料。方法抽取我院消化内科2020年第四季度的门诊处方(9948张)进行回顾性分析。结果我院消化内科常用的抗抑郁药有帕罗西汀、舍曲林、艾司西酞普兰、度洛西汀、曲唑酮、阿戈美拉汀和氟哌噻吨美利曲辛。使用抗抑郁药处方共1295张,使用率为13.02%,女性多于男性,45~64岁患者居多。其中反流性食管炎216张、慢性胃炎175张、肠易激综合征171张、消化不良147张及十二指肠溃疡103张,共占62%。使用频率最高的抗抑郁药是氟哌噻吨美利曲辛(88.19%)。以单一抗抑郁药为主,联用两种抗抑郁药、联用抗精神病药处方分别占2.39%和2.01%。不合理处方主要是遴选的药品不适宜(8张)和联合用药不适宜(3张)。结论消化内科抗抑郁药的临床使用基本合理,但处方用药存在一些问题,主要是遴选药物不适宜和联合用药不适宜。重点关注18岁以下的青少年、联合用药及氟哌噻吨美利曲辛的使用率,应加强药学监护及用药教育,以确保临床用药的合理性及安全性。OBJECTIVE Analyze the application and rationality of antidepressants in Department of Gastroenterology and provide the references of antidepressant medication and pharmaceutical care in Department of Gastroenterology.METHODS Randomly choose and analyze the outpatient prescriptions(9948 pcs)from Department of Gastroenterology of our hospital in the fourth quarter of 2020.RESULTS Paroxetine,sertraline,escitalopram,duloxetine,trazodone,agomelatine,flupentixoton-meritracine were the antidepressants used in Department of Gastroenterology in this study.1295 prescriptions used the antidepressants,with a rate of 13.02%,more women than men.And most of the patients were from 45 to 64 years old.Among them,there were 216 pcs of reflux esophagitis,175 pcs of chronic gastritis,171 pcs of irritable bowel syndrome,147 pcs of dyspepsia and 103 pcs of duodenal ulcer,accounting for 62%.The antidepressant most frequently used was flupentixol-meritracine(88.19%).Most of them used the single antidepressants,and the rate of combination of two drugs including antipsychotics accounted for 2.39%,combined use of antipsychotics accounts for 2.01%.Irrational prescriptions mainly included unsuitable selected drugs(8 pcs)and unsuitable combined drugs(3 pcs).CONCLUSION The clinical use of antidepressants in Department of Gastroenterology is basically reasonable,but there are some problems.More attention should be payed to the young people under the age of 18,combined medication and the use of flupentixol-meritracine.Pharmaceutical care and medication education should be strengthened to ensure the rationality and safety of clinical medication.
