全科医师家庭签约服务固化关系模式对社区高血压管理的影响

摘　　要：目的:探讨社区高血压管理采用全科医师家庭签约服务固化关系模式对干预效果的影响。方法:选取2019年3月-2021年3月上海市黄浦区小东门街道社区卫生服务中心收治的1500例高血压患者,采用随机摸球法分成观察组750例与对照组750例,对照组采用常规社区管理干预,观察组采用全科医师家庭签约服务固化关系模式干预,对比两组血压水平、健康知识掌握情况、满意度及生活质量。结果:干预后两组血压水平均降低,且观察组低于对照组,差异有统计学意义(P<0.05);干预后两组健康知识评分提高,观察组评分较对照组更高,差异有统计学意义(P<0.05);观察组满意度较对照组更高,差异有统计学意义(P<0.05);干预后两组生活质量综合评定问卷-74评分提高,观察组评分较对照组更高,差异有统计学意义(P<0.05)。结论:全科医师家庭签约服务固化关系模式应用于社区高血压管理中,可改善其血压水平,加深患者对高血压相关知识的了解,提高其满意度与生活质量。Objective:To explore the influence of family contract service solidified relationship model of general practitioner in community hypertension management on the intervention effect.Methods:A total of 1500 patients with hypertension who were admitted in the Community Health Service Center,Xiaodong Men Street,Huangpu District from March 2019 to March 2021 were selected and divided into observation group(n=750)and control group(n=750)by random ball-touching method.The control group received routine community management intervention,and the observation group received general practitioner family contract service solidification relationship model intervention.Blood pressure,health knowledge mastery,satisfaction and quality of life were compared between the two groups.Results:After intervention,the blood pressure level in the two groups were decreased,and the level in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).After intervention,the health knowledge score of the two groups was improved,and the score of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The satisfaction of observation group was higher than that of control group,and the difference was statistically significant(P<0.05).After intervention,the score of generic quality of life inventory-74(GQOL-74)was improved in the two groups,and the GQOL-74 score of observation group was higher than that of control group,and the difference was statistically significant(P<0.05).Conclusion:The application of the family contract service solidified relationship model of general practitioners in community hypertension management can improve patients'blood pressure level,deepen patients'understanding of hypertension related knowledge,and improve their satisfaction and quality of life.
