摘 要:MYCN基因在神经母细胞瘤(neuroblastoma,NB)细胞生长、凋亡、分化、浸润、转移及血管形成等方面发挥重要作用。临床中,MYCN基因扩增是NB危险度分级的重要指标之一。然而,MYCN基因检测面临检测方法操作复杂、检测成本高及不易获得瘤组织细胞等问题。研究表明,NB患儿血清循环MYCN基因扩增情况与瘤组织中一致,血清循环MYCN基因扩增情况作为NB预后及复发监测指标具有灵敏度高、特异性好、操作简便及无创等优势。因此,血清循环MYCN基因扩增检测有望替代瘤组织中MYCN基因扩增检测,用于NB预后及复发监测。本文通过介绍NB患儿血清循环MYCN基因扩增情况在NB诊断、预后及复发监测中的应用研究进展,并对其未来发展进行展望,以期推进其临床转化应用的速度。MYCN gene plays an important role in proliferation,apoptosis,differentiation,tumor infiltration,metastasis and angiogenesis of neuroblastoma cell.Clinically MYCN gene amplification in cancerous tissue is one important indicator for risk grading of neuroblastoma.However,MYCN detection in cancerous tissues has remained problematic,such as complicated procedures of detection methods,high cost of detection and difficulties of harvesting cancerous tissues.Numerous studies have demonstrated that amplification status of MYCN gene in sera of neuroblastoma is consistent with that in tumor tissue.Seral MYCN gene amplification status offers the advantages of high sensitivity,excellent specificity,simple operation and non-invasiveness as a prognostic and recurrence indicator for neuroblastoma.Therefore seral MYCN gene amplification status is expected to replace MYCN gene amplification status in cancerous tissues and monitor the prognosis and recurrence of neuroblastoma.The applications of MYCN gene amplification status in the diagnosis,prognosis and recurrence monitoring of neuroblastoma were summarized.And future developments of MYCN gene amplification status in neuroblastoma prognostic assessment and recurrence monitoring were discussed for accelerating the pace of clinical transformations.
