摘　　要：目的:探讨术前血清基质金属蛋白酶-8(MMP-8)及基质金属蛋白酶抑制因子-1(TIMP-1)水平在老年结肠癌根治术预后中的意义。方法:收集2016年1月—2017年12月在郑州人民医院行结肠癌根治术的老年结肠癌患者164例,检测其术前外周血清的MMP-8和TIMP-1水平并分析其与患者临床资料的相关性,采用单因素和多因素回归分析血清MMP-8和TIMP-1水平与患者无进展生存期(PFS)及总生存期(OS)的相关性。结果:不同MMP-8组间肿瘤分化程度及TNM分期有统计学差异(P<0.05),不同TIMP-1组间淋巴结转移比例及TNM分期有统计学差异(P<0.05)。低MMP-8组的PFS和OS均明显高于高MMP-8组(χ^(2)=6.521,P=0.011;χ^(2)=7.407,P=0.006);低TIMP-1组的PFS和OS均明显高于高TIMP-1组(χ^(2)=12.33,P=0.000;χ^(2)=5.768,P=0.016);高MMP-8水平是患者PFS的独立危险因素,而高TIMP-1水平是患者OS的独立危险因素(P<0.05)。结论:术前外周血中MMP-8及TIMP-1水平均与老年结肠癌根治术患者预后有一定相关性,可作为潜在的预后评估有效指标。Objective:To investigate the significance of perioperative serum matrix metalloproteinase-8(MMP-8)and tissue inhibitor of metalloproteinase-1(TIMP-1)levels in the prognosis of elderly patients with colon cancer after radical operation.Methods:164 elder patients with colon cancer who underwent radical resection in People’s hospital of Zhengzhou from Jan.2016 to Dec2017 were collected.The MMP-8 and TIMP-1 levels in preoperative serum were detected and the correlation with the clinical data was analyzed.Univariate and multivariate regression were used to analyze the correlation between MMP-8 and TIMP-1 and progression free survival(PFS)and overall survival(OS).Results:There were significant differences in tumor differentiation degree and TNM stage between low and high MMP-8 groups(P<0.05),and there were statistically significant differences in lymph node metastasis rate and TNM stage between low and high TIMP-1 groups(P<0.05).Survival analysis showed that PFS and OS in the low MMP-8 group were significantly higher than those in the high MMP-8 group(χ^(2)=6.521,P=0.011;χ^(2)=7.407,P=0.006;respectively),and The PFS and OS of the low TIMP-1 group were significantly higher than those of the high TIMP-1 group(χ^(2)=12.33,P=0.000;χ^(2)=5.768,P=0.016;respectively).High MMP-8 was an independent risk factor for postoperative PFS(P<0.05),while high TIMP-1 was an independent risk factor for postoperative OS(P<0.05).Conclusions:The MMP-8 and TIMP-1 levels in preoperative peripheral serum were correlated with the prognosis of elderly patients undergoing radical resection of colon cancer,which could be used as potential effective indicators for prognosis evaluation.
